Under the direct supervision of the supervising RN the PACE Licensed Vocational Nurse (LVN) assists in assessing for and delivering clinical skilled services. This position works closely with all PACE departments including the Clinic Day Health Center Home Care and supports PACE activities including on the supervisions of Personal Care Assistants (PCAs) delivering assistance with Participant activates in the Center and Home environments as assigned.

- Screen potential participants in the clinic day center or home (and or virtually) and assists the Supervising RN in assessing their environment personal care needs of the frail and elderly population and identify and deliver skilled needs.
- Gather compile and submit all information during Participant encounters.
- Under supervision by the Supervising RN assists in completing level of care tool including diagnoses medications ADLs (Activities of Daily Living) IADLs (Instrumental Activities of Daily Living) in addition to a narrative summarizing potential participants skilled needs.
- Track whether assessments have been completed.
- Review medical records for each participant ongoing services and services received within past year using EMR plan of care for the upcoming year using EMR and IDT annual assessments to assist in ensuring continuing skilled needs are identified.
- Assist the Supervising RN with current medical status and functional capabilities for each participant.
- Ensure the LOC recertifications are completed in a timely manner as per DHCS protocol and assists with audits and follows up as appropriate.
- Basic wound care and vitals.
- Administration of immunizations injections and medication as prescribed within scope.
- Ensure documentation is complete; document medical history within the system keep medical record current and upload as needed.
- Supervise Patient Care Assistants in accordance with regulations.
- Assess patients physical and mental wellbeing report to MD or RN.
- Attend and participate in staff meetings inservices projects and committees as assigned.
- Be flexible in schedule of hours worked.

Physical Demands
- Ability to lift up to 30 pounds. Moving lifting or transferring patients may involve lifting or pushing greater than 30 pounds should be done with assistance as appropriate.
- Ability to exert maximum muscle force to lift push pull or carry patients.
- Must be able to hear staff/clients on the phone and those who are served inperson and speak clearly in order to communicate information to clients and staff.
- Must have vision with or without lenses that is adequate to read memos a computer screen personnel forms and clinical and administrative documents
- Must have high manual dexterity.
- Must be able to reach above the shoulder level to work must be able to bend squat and sit stand stoop crouching reaching kneeling twisting/turning fingering and feeling.
Work Environment
- Exposure to biohazards including infectious material and waste and any other conditions common in a health care environment.
- Subject to unpleasant odors.
- The noise level is usually quiet to moderate but may at times be noisy and crowded.
Education/Experience Requirements
Minimum Qualifications
- Valid CA Drivers License acceptable driving record and vehicle insurance.
- Drive to participants homes as needed.
- Develop rapport establish and maintain effective relationships with internal and external contacts at all levels and from varying cultural and ethnic backgrounds.
- Ability to work under stressful conditions.

- Knowledge of nursing best practices MediCal Medicare and community engagement a plus available community resources.
- Proficient in Microsoft Office applications.
- Maintains professional affiliations and any required certifications.
- Bilingual English/Spanish preferred.
Education/Experience
- High school diploma or equivalent.
- Graduate of an accredited school of professional nursing.
- Current unencumbered Licensed Vocational Nurse (LVN) in the state of CA.
- Current BLS CPR Card certified by American Heart Association.
- Minimum one 1 year direct patient care experience with Home or Community Health Provider experience within the last three 3 years required.
- Minimum one 1 year experience working with the frail or elderly population
