Avanti Home Cares Registered Nurse (RN) Case Managers implement evaluate and supervise the care of clients in their own homes. Avanti Home Care RNs work as a collaborative part of the health care team. Providing total client care to include; assessments organizing caseload coordination of client records oversees field staff medications and treatments administered as ordered by the physician physical and psychological care and client and family education. Assesses and interprets symptoms and reports significant changes in clients condition.
Qualifications:
- Familiarity with medical software and equipment and high comfort level with computers and other forms of technology
- Ability to build rapport with patients
- Strong problem solving and critical thinking skills
- Ability to selfdirect selfschedule and work independently
Avanti staff enjoy flexible schedules paid travel and mileage and the opportunity to work some hours from home.
Job Type: Fulltime
Pay: Flexible and range from $30.00 per hour

- Rotating oncall
