Job Description
Job Summary:
The Acute Care Hospital At Home (ACHAH) NP/PA will provide care and oversight for patients enrolled in ACHAH which includes inhome & telemedicine visits as well as inpatient rounding where you will provide education and interface with primary teams patients and families. Responsibilities include significant interdisciplinary collaboration providing expert level clinical care & coordination of care via a full range of services that will improve quality of patient care assessing & evaluating patients ordering appropriate laboratory testing & diagnostic studies arranging for referrals testing consultation & therapeutic services and maintaining outstanding relationships with families referring physicians & all members of the health care team.
Minimum Qualifications:
NYS licensed/registered NP or PA required. Masters Degree as a Nurse Practitioner or Physician Assistant & current National Certification consistent to the clinical practice also required.NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire).
CPRBLS Certification required at the time of appointment.
Preferred Qualifications:
12 years NP/PA experience in Medicine Acute Care preferred.
Work Days:
Monday Friday Days with weekend and holiday rotation.
