DPV Health is expanding the Pathway to Good Health service for vulnerable children and young people residing in Out of home Care in partnership with the Department of Health Victoria and DFFH Child Protection. As part of the statewide rollout of the program DPV Health provides targeted health care across the Northern Metropolitan Melbourne region with clinic locations in Broadmeadows and Mill Park as well as community inreach and outreach.

The Nurse Navigator is responsible for referral management triage health history collation and identification of unmet health needs for children and young people in care. The position supports health system navigation and coordinates care in partnership and collaboration with members of a childs care team to support improved health outcomes. The incumbent will use clinical expertise and a trauma informed approach to contribute to comprehensive health assessments and production of holistic Health Management Plans as part of a multidisciplinary team.

Working as part of the Pathway to Good Health multidisciplinary team you will:

  • Act as an interface between the Child Protection system and health services ensuring cross sector collaboration to provide health system navigation.
  • Conduct secondary consultation with DFFH Child Protection North Division and other partners to provide clinical triage identify unmet health needs and provide individualised clinical advice.
  • Gather health and social information in line with information sharing legislation to collate health summaries utilise relevant records to interpret review and communicate clinical information.
  • Identify and navigate barriers to service engagement by offering a flexible model of care.
  • Collaborate with the multidisciplinary team to contribute to comprehensive health assessment and production of holistic health management plans aligned with the National Clinical Assessment Framework for Children and Young People in Out of Home Care.
  • Conduct referral management and care coordination to ensure timely facilitation of multidisciplinary assessment and initiation of referral to other health services as appropriate.
  • Participation in multidisciplinary clinical case conferences and care team meetings with external partners involved in a clients care team to proactively promote collaborative care.
  • Provision of health promotion health education psychoeducation advocacy social prescribing and support for clients and their caregivers.
  • Works in partnership with clients and caregivers to build capacity and health literacy ensuring clients are engaged in shared decision making about their health goals and care planning.

To be successful in this role you will bring:

  • Current APHRA Registration as Division 1 Registered Nurse
  • Paediatric/ adolescent health nursing experience of at least three years postregistration post graduate qualification in speciality area highly desirable.
  • Experience working with vulnerable children adolescents and clients from culturally diverse backgrounds desirable.
  • Welldeveloped paediatric clinical assessment skills and a sound understanding of child development and the social determinants of health.
  • Ability to work within a highly sensitive environment with an understanding of the effects of trauma and the principles of trauma informed care.
  • A positive regard for and a commitment to supporting children and young people facing adversity.
  • Demonstrated ability to support a coordinated approach by working with a range of stakeholders to improve health outcomes.
  • Ability to work autonomously within scope of practice to meet agreed timelines as well the ability to work within an interprofessional multidisciplinary framework.

  • Successful candidates will be required to undergo a National Police Check International Police Check (if applicable) NDIS clearance and hold a valid WWCC.
  • Full driver license and travel between sites required.
