Job Description
ATTENTION: This position requires graduation as a Doctor of Physical Therapy (or equivalent standing) and a state license as a Physical Therapist.

Location: Home Care Commack NY
Type: Fulltime Parttime or PRN (Minimum of 15 hours per week and 3 weekdays of availability required)
Position Overview:
We are looking for a Physical Therapist to join our team in the Commack NY area. This role involves providing outpatient therapy in homebased settings primarily to the geriatric population. Candidates can work fulltime parttime or PRN with a minimum commitment of 15 hours per week.

Job Requirements:
- Graduation as a Doctor of Physical Therapy (or equivalent).
- Eligibility for New York state licensure as a Physical Therapist.
- Availability to work a minimum of 15 hours per week.
- Regular and consistent weekly availability required including 3 weekdays.
- Strong professionalism accuracy and dependability.

- For PRN positions prior experience is preferred.
