Provides personal care services under the direction of the RN/Case Manager. Assigned to specific clients and performs services for clients as necessary to maintain their personal comfort.

Essential Functions:
- Read and record temperature pulse and respiration
- Patient hygiene bathing shampoo skin/nail care oral hygiene shaving dressing toileting
- Maintain a clean safe and healthy home environment meal preparation laundry light housekeeping
- Follow written assignment and instructions
- Complete and submit appropriate documentation
Qualifications:
- Active certification as a CNA in the state(s) of practice
- At least eighteen 18 years of age
- Current CPR certification and TB test
Standard Rate: $13.50 $16.00 Hourly
