Licensed Practical Nurse/Licensed Vocational LPN/LVN Nurse Job Summary
The Licensed Practical/Vocational Nurse LPN/LVN provides skilled nursing services to patients in a variety of healthcare settings. The Licensed Practical Nurse/Licensed Vocational Nurse LPN/LVN is responsible for working with other healthcare professionals to help treat people with various injuries illnesses or disabilities.

Licensure
Current and unencumbered Licensed Practical Nurses/Licensed Vocational Nurses LPNs/LVNs license in the state of practice.
Experience
At least one year of experience in the last 3 years.

- Hepatitis B Statement or Declination
- TB Screening
- Current Physical Exam
- MMR and Varicella immunizations or titers
