Job Description
Job Summary: We are looking for a LPNto join our growing team! The right candidate will have a current state license as LPN and have a friendly attitude along with excellent communication skills.

Responsibilities
Complete initial and ongoing assessments of all individuals of 18person caseload
Assess patients conditions as needed
Send chart observations and perform evaluation tasks including vital signs and medication reviews
- Assist PA with monthly clinics on site
Oversee medicationadministration by unlicensed personnel
Assist with quarterly Psychiatric clinic
Ensure appropriate medical services are provided for all IDDindividuals
Assist with medical appointments
Provide effective communication to patients/clients their family members team members and other health care professionals
- Complete Workers Compensation paperwork and follow up as needed
Qualifications
Current state license as a LPN
Proof of current CPR certification
Compassionate friendly demeanor
Excellent communication skills
Current drivers license and reliable transportation
Ability to work a flexible schedule
On call availability
