Job Description

The Physical Therapist is a registered professional who provides skilled physical therapy services on an intermittent basis to clients in their homes. These services are performed in accordance with physicians orders.

The rate is pay is $80 per evaluation and $70 per follow up.

PRIMARY FUNCTIONS

  • Assists in the admission of the client to the Agency by:
    • Initial ongoing assessment to determine level of functioning include OASIS assessments at appropriate time points.
    • Obtaining medical history particularly as it relates to the present condition.
    • Interpreting Physical Therapy Agency services to client and families as a part of planning for care.
    • Assuring appropriateness of physicians orders and discussing necessary changes.
  • Makes arrangements for outpatient services when appropriate.
  • Evaluates the effectiveness of Physical Therapy intervention and updates the client care plan as needed.
  • Instructs clients and their families in the use of prosthetic orthotic and assistive devices (canes walkers wheelchairs etc).
  • Instructs the family in the clients total physical therapy program.
  • Records & reports to the supervisor and physician the clients reaction to any treatment and/or any changes in the clients condition. Communicates with the supervisor and physician at least whenever changes occur regarding client status and obtains additional orders if necessary.
  • Communicates with other team members(including Home Health Aides) and when appropriate instructs them in certain phases of physical therapy that they may use while working with the client.
  • Prepares an assessment of the clients condition and periodic evaluations and summaries.
  • Develops a plan of care to include interventions and goals
  • Writes clinical/progress notes on the date of the visit and incorporates the same in the clients record within 72 hours.
  • Supervises the Home Health Aide every two weeks when physical therapy is the only professional discipline involved in the clients care. Supervises the Physical Therapy Assistant at least once monthly as outlined in the PTAs job description.

QUALIFICATIONS
(EDUCATION and/or EXPERIENCE)

  • Associates degree or equivalent from twoyear college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.
  • 2 years Physical Therapy experience.
  • Florida Physical Therapist License and CPR Certification
  • Valid drivers license and automobile insurance in the State of Florida.
  • Experience with the frail and elderly.
  • Reliable transportation to meet visit schedule
  • Ability to use equipment with visual and auditory mechanisms.
  • Ability to effectively communicate in English.
  • Flexible schedule to meet the needs of participants.
  • Ability to physically access any home i.e. navigate stairs and narrow spaces tolerate heat and lack of air conditioning in any area Company serves.
  • Ability to perform the essential functions and physical requirements (including but not limited to: lifting participants and/or equipment bending pushing/pulling kneeling) of the job with or without reasonable accommodation.
