Job Description

QUALIFICATIONS:
  1. Graduate of an approved school of practical nursing.
  2. Current license in the Commonwealth of Massachusetts.
  3. Membership in representative practical nursing organization is encouraged. Participation in continuing education is required.
  4. Must maintain annual competencies as related to practice protocol.

SKILLS/ABILITIES/COMPETENCIES REQUIRED:
  • Must have necessary analytical judgmental and interpersonal skills to deal effectively with patients families and coworkers.
  • Communicates with physicians to determine nursing needs and to solve problems regarding nursing issues.
  • Responds to requests for change that will improve practice conditions for physicians either by implementing such changes or by discussing them with the Practice Manager.

WORKING CONDITIONS:
  • Normal patient environment where there are some exposures to communicable diseases unpleasant odors needles and blood products.
  • Requires lifting transporting and positioning of patients and/or equipment which could result in musculoskeletal strain or injury.
  • Requires adjustment of equipment which could result in minor cuts to fingers and hands.
GENERAL SUMMARY/OVERVIEW STATEMENT:

This Licensed Practical Nurse will assist in the clinical practice for one or more medical specialties within the Department of Medicine. Reports to the practice manager for daytoday operational issues and reports to the MD Clinical Director for technical clinical direction and supervision. Responsible for all nursing functions necessary for the complete care and efficient management of patients through the ambulatory facility including teaching and assisting patients with needs or problems relevant to their visit as well as assisting physicians. Provides treatment instructions teaching for patients upon the directive from providers. He/she must be comfortable working with patients who may have infectious conditions such as HIV/AIDS Hepatitis TB and STDs who are from a variety of ethnic and cultural backgrounds different sexual orientations and practices. He/she must have the desire to learn and work well with others. This position requires flexibility and the willingness to provide support and assistance throughout all Medicine Specialties when called upon. He/she must maintain current licensure and continuing education in accordance with state and BWH policies. Must meet and adhere to scope of practice and state licensure guidelines in accordance with DPH Joint Commission and the board of nursing regulatory policies


PRINCIPAL DUTIES AND RESPONSIBILITIES:

Fulfilling Responsibilities to Patients
  1. The primary responsibility of this position is phone screening and communication with patients and providers. Other patient contact referred to is minimal but required when requested.
  2. Calls patients from waiting room and provides them with proper instructions or information. Is responsible for managing the flow of the clinic session. Communicates with clinic coordinator nurse social workers fellows PA NP and attending physicians to expedite all patients receiving necessary care in the acceptable waiting time.
  3. Accurately performs vital signs procedures electrocardiogram or other required tests. Documents care provided in electronic record as appropriate.
  4. Assists patients in a manner that promotes the perception that the staff is courteous efficient and interested in their health care.
  5. Treats all patients with dignity and professionalism paying special attention to confidentiality. This includes written spoken or electronic communication. Maintains a professional relationship with the patient.
  6. Accurately records all patient data and maintains all necessary records or files relating to clinical patient care issues.
  7. Coordination of care for patient (home care test prep lab prep)
  8. Enters prescription refills through EPIC queue and faxes received. Also responsible for obtaining prior authorizations when needed for medication or ancillary testing.
  9. Is able to communicate effectively normal test results.
  10. Is ability to perform dressing changes as directed by provider
  11. Transports or escorts patients and families to other areas within the hospital if necessary
  12. Is able to perform teachings as directed by provider
  13. Responsible for POC testing administrating and documenting injections
Fulfilling Responsibilities to Physicians
  1. Assists physicians during examination procedures & care of the patient following all BWH infection control procedures.
  2. Assists physicians in the acquisition and proper use of all equipment and supplies relating to the care of the patient.
  3. Accurately communicates messages form patients to providers and accurately communicates the physician instructions to the patient.
  4. Maintains a professional relationship with the physicians.
  5. Keep physicians up to date of patient volume and waiting status during appointment sessions.
  6. Communicates with patients regarding delay in physician schedules.
  7. Submits prescription renewal requests via LMR Rx queue. Calls or faxes in prescriptions after approval by and as instructed by provider.
  8. Administers medications and vaccinations as ordered by the provider in accordance with BWH policy. Obtains medications and vaccines from the pharmacy as required. Documents the same day and in accordance with BWH policy.
  9. Maintains and ensures patient confidentiality. This includes written spoken or electronic communication.
Clerical/Documentation Responsibilities
  1. Completes physicians orders accurately and thoroughly for tests and appointments to be scheduled elsewhere.
  2. Obtains prior authorizations for medications and radiology tests.
  3. Is able to complete to the best of their knowledge in form completion
  4. Accurately provides documentation of patient care in patient medical record as required
Equipment and Supplies
  1. Keeps all patientcare and procedure equipment clean and in good working order and reports equipment problems to Manager. Checks code cart daily. Checks and records temperature of refrigerator and freezer daily. .
  2. Keeps inventory of necessary clinical and administrative supplies.
  3. Makes sure that all supplies and forms are properly stocked in patientcare and administrative areas.
  4. Keep all patientcare areas neat and in accordance with BWH BWPO DPH and JCAHO standards.
  5. Routinely checks all supplies and medications to ensure that they are not out of date.
  6. Maintains a safe working environment.
  7. Responsible for ensuring that all equipment is checked annually by biomedical engineering department.
Interacting with Fellow Employees
  1. Works to establish and maintain good relationships with fellow employees.
  2. Works cooperatively with fellow employees to solve problems and to make the Department function efficiently.
  3. Fills in for fellow employees when asked to do so or when the need arises.
  4. Takes initiative in organizing supply area reporting computer telephone equipment malfunction safety violations and general repairs to the practice manager when discovered.
  5. Arrives for work promptly reports to supervisor upon arrival when leaving for and returning from breaks and at the end of the shift. If necessary for patient care and safety stays beyond end of shift to assist physicians and nurse.
  6. Maintains voicemail in accordance with policy. Checks voicemail and email at least every two hours. Places out of office messages on both before scheduled absences from work.
Interacting with supervisory or Administrative Staff
  1. Follows instructions of and assists supervisory staff as required.
  2. Maintains good communication with and reports problems to supervisors.
  3. Attends all required staff meetings.
  4. Adheres to JCAHO regulations
Other
  1. Performs other duties and tasks as requested by the provider and practice manager to ensure the efficient overall performance of the practice.
