Job Description

GENERAL SUMMARY/ OVERVIEW STATEMENT:

The Staff Physician Assistant will work with the Interventional Cardiovascular Division of the Department of Medicine. This individual will assist the attending cardiologist in all areas of patient care in the Interventional Cardiovascular Medicine Service.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Clinical Duties:
Preprocedure Patient Care: The Staff Physician Assistant will be responsible for obtaining and coordinating the following general items: history and physical laboratory results charting of clinical findings admission orders nursing instructions and other required elements. The Staff Physician Assistant in collaboration with the Laboratory Flow Managers will coordinate admissions and transfers to the hospital and ensure that informed consent for procedures is obtained. The Staff Physician Assistant will perform preprocedure assessments in the Preadmission testing center Recovery room/holding area or outpatient setting as needed.

Interface between Cardiac Catheterization and Intervention Laboratory and Recovery Area:The Staff Physician Assistant will be knowledgeable about the Cardiac Catheterization and Intervention Laboratory scheduling procedure room turnover and availability of resources. He/she will be called upon to manage patients after their procedures and while they await transfer to patient floor and will assure availability of equipment for special procedures. The Staff Physician Assistant will ensure blood bank preparation and formal or standing surgical backup when required for patient care. The Staff Physician Assistant will assist in family liaison.

Medical Treatment and Postprocedure Care: The Staff Physician Assistant will be responsible for many aspects of care under the supervision of the attending physician including but not limited to: fluid and electrolyte management nutrition pain management transfusion therapy anticoagulation management education and postdischarge planning. Following procedures the Staff Physician Assistant will manage vascular access monitor and manage arrhythmias pacing drug therapy O2 therapy and other noncardiac medications under the supervision of the attending physician. The Staff Physician Assistant will assist in the evaluation and management of clinical events such as fever altered hemodynamics CHF pulmonary complications abrupt vessel closure arrhythmias access site complications and others. The Staff Physician Assistant will assist in transfers to and from the intensive care units.

Procedure responsibilities: The Staff Physician Assistant will assist the attending physician during diagnostic interventional and research procedures performed in the Cardiac Catheterization and Intervention Laboratory and other clinical areas.

Discharge Planning: The Staff Physician Assistant will perform patient education regarding discharge including preventive care and risk factor reduction. He/she will write discharge orders prescriptions and dictate discharge summaries in collaboration with the attending physician. The Staff Physician Assistant may act as a liaison between the attending cardiologist and family members and will schedule followup care as necessary including VNA if needed.

Research and Education: The Staff Physician Assistant will be offered the opportunity to participate in aspects of research involving planning performance and analysis of data and publications. The Staff Physician Assistant will complete the 100 hours of continuing medical education necessary during each 2year period of employment. The Staff Physician Assistant will attend internal and external education meetings and read appropriate professional journals in order to maintain and enhance skills and clinical competence. The Staff Physician Assistant will be encouraged to advance his/her medical and technical skills as appropriate to deliver optimum patient care.

QUALIFICATIONS:

Must have successfully completed a Graduate Level accredited Physician Assistant Program approved by AMA Board of Approval and Certification of PA programs. Must have 4 years of college level education and bachelors degree or equivalent. (BS/MS joint programs are also acceptable; BS with PA certificates are acceptable for graduates before 1995. Board Certification by the National Commission on Certification of PAs is required. Cardiology experience preferred. Specific license/certification/registration required to perform the job. NCCPA certified License as a Physician Assistant in Massachusetts Maintain current DEA and Massachusetts Controlled Substance Licenses Complete 100 hours of CME according to national and state regulations during each 2 year period of employment. Maintain current BLS/ACLS certification Complete BWH Professionalism Program within 3 months of date of hire. Complete BWH Procedural Sedation Training if determined necessary.

SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:

Each PA shall demonstrate the following professional knowledge and skills: Proficient with BWH computer systems as needed to independently perform daily tasks such as order entry EMAR lab tests and results incident reporting BICS etc. Consistently provide high quality and timely documentation including admission notes progress notes discharge summaries and dictations. Must have knowledge of billing practices. PAs: Must be energetic quality driven and have proven ability to be a productive member of a cohesive team of peers dedicated to providing optimum patient care. Must exercise the highest degree of professional judgment under the direct supervision and guidance of a licensed physician. Must maintain a professional attitude towards patients and exercise such mature independent originality in solving problems not covered by existing guidelines within legal limits to meet the needs of the patients. Must maintain a professional attitude towards patients at all times and an excellent and collegial working relationship with other personnel involved in patient care. Must demonstrate understanding of procedures policies and documentation required to ensure compliance with hospital standards of care. Each PA shall demonstrate the following clinical knowledge skills and competencies: For the most common diagnoses admitted to the Interventional Cardiology Service demonstrate competency to effectively diagnose formulate appropriate treatment plan and efficiently implement the plan without much direction and with a greater level of independence from the attending physician. Clinical skills and knowledge as well as professionalism of the PA candidate are assessed by Interventional Cardiology Attending under the direction of the Cath Lab Medical Director. Candidates with two or more years of relevant PA experience prior to joining the service become eligible for a preliminary evaluation of competencies approximately 3 months after joining the service. The purpose of the preliminary evaluation is to provide feedback to the candidate on progress towards level of competency required and provide guidance on additional development needs. After approximately 6 months after joining the service the candidate then becomes eligible to complete the assessment and if successful advances to a PAII. If a candidate does not pass the competency assessment cath lab attending MDs will provide feedback on additional development needs. Reassessment is possible after 6 months of focused development. Competencies are assessed by cath lab attendings either through direct observation or verbal communication. The attending physician(s) performing the assessment will complete a competency assessment. The candidate is responsible to provide example cases which he or she has cared for patients with the most common diagnosis.

WORKING CONDITIONS:

Work requires a considerable amount of walking and standing while wearing heavy lead. Requires physical effort to lift and position patients and radiographic equipment. Exposure to some radiation during fluoroscopy spot filming and digital acquisition in labs. Exposure to blood and waste products and contagious diseases. Constant changes in lighting overhead lighting followed by dark during flouroscopy bright lights on illuminators and surgical lamps dark in the reading room etc. Stressful highvolume high acuity patient care environment.
