GENERAL SUMMARY/OVERVIEW STATEMENT:
The Physician Assistant I Per Diem is a member of the Division of Otolaryngology team and fills the role of the inpatient and consult Physician Assistant on a per diem basis. The nature and scope of practice of the Physician Assistant I is to assess to diagnose and to treat health problems of Patients across the continuum of care in collaboration with the attending Physicians. The Division of Otolaryngology is responsible for the care of patients in recovery on inpatient floors across the hospital as well as providing Otolaryngology consult services to other hospital divisions and departments. The practice will encompass patients and their families who require surgical treatment.
CLINICAL DUTIES:

a. Team Work Interpersonal & Communication Skills
i. Communicates and implements plans to all team members with medical judgment to adjust plans as clinically indicated (ALL changes in plans will be discussed with Attendings and Consult input will be communicated to the Attending prior to implementation)
ii. Demonstrates thorough knowledge of patients in his/her care and is prepared to discuss treatment plans clinical condition and all relevant information with Consulting Staff team members and Attending in an organized and concise fashion.
iii. Is approachable and professional towards patients family members and all hospital staff at all times
b. Quality of Care Patient Care & Medical Knowledge
i. Provides detailed explanations of all surgical procedures performed on the service to team members students and patients including risks and benefits. Informed consent can be performed under the direction of the Attending Physician.
ii. Prioritizes patient care above all other tasks at all times.
iii. Continuously works to improve patient care and increase patient safety
iv. Follows all guidelines for infection control and precautions.
v. Demonstrates and maintains a thorough knowledge of anatomy and surgical techniques with continued study of the Otolaryngology PA Manual and current articles

2. Performs patients admitting histories and physicals; records information accurately in patient records in a timely fashion; communicates information to resident and attending teams; provide consultation and diagnostic procedure services to patients on various service lines.
3. Performs routine preoperative and postoperative procedures e.g.: insertion/removal of tubes lines sutures; performs postoperative wound care
4. Examines and evaluates patients and assists in development of their treatment plan with resident physicians and attending surgeons
5. Appropriately responds to emergency situations; manages patient issues as they arise to the extent that the PAI will obtain and evaluate studies/labs write orders obtain consultations and interpret lab values and xrays with the assistance of the supervising physicians as necessary
6. Provides age specific care to surgical patients from early adolescence through the geriatric ages
7. Will work with physician trainees and will alternate with them in scheduled shifts for patient care
8. Will work under the direction of supervising critical care attending and fellows
9. Provides teaching to medical students and physician assistant students and other health care personnel as appropriate
10. Stays uptodate on the latest literature and developments in the field: attends national and local conferences reviews journal updates actively participates in divisional conferences attends physician assistant departmental meetings seeks out additional learning opportunities.
While PAs can expect their work schedules to be relatively predictable planned far enough in advance whenever possible they should also expect variation in their schedules as needed to cover the clinical needs as they become apparent to the department/division/clinics. PAs may be asked to take call from home to pick up additional shifts as needed to work nights and weekends and to provide backup coverage all as part of their work expectations in this job all with reasonable notice and in compliance with hospital compensation guidelines.

b. Facilitates communication between team members in clinic on the floors and in the operating room by utilizing any available resources (email pager cellular phone land line and direct face to face communication and transfer of information whenever possible).
c. Acts as a liaison between staff and patients communicating updates treatment plans followup care testing etc in a timely and precise manner.
d. Discusses ideas and concerns with other team members in a positive feedback forum that focuses on improving patient care.

4. Operations Support Administrative Responsibilities

c. Continues to provide the highest standard of patient care while incorporating costcutting measures and improving efficiency within the Division and the hospital.
d. Provides clinical support and administrative oversight for Attending Physicians to improve patient care and satisfaction.
QUALIFICATIONS:
1. License/Certification/Registration:
Licensed as a Physician Assistant in Massachusetts
Maintain current DEA and Massachusetts Controlled Substance Licenses
NCCPA certified
Applicant is to be Board certified or eligible by the National Commission on Certification of Physician Assistants
Complete 100 hours of CME according to national and state regulations during each 2year period of employment
Complete BWH Professionalism Program within 3 months of date of hire
Employment is contingent on all completed credentialing requirements.
2. Education:
Must have successfully completed a Graduate Level accredited Physician Assistant Program approved by AMA Board of Approval and Certification of PA programs.
Must have 4 years of college level education and bachelors degree or equivalent. (BS/MS joint programs are also acceptable; BS with PA certificates are acceptable for graduates before 1995.
Board Certification by the National Commission on Certification of PAs is required.
3. Work Experience: Each PAI shall meet the following requirements for work experience:
12 months relevant full time experience as a PA urgent care experience preferred.
12 years relevant full time experience as a PA in this specialty preferred.
Other previous Hands on patient care experience may be considered at the discretion of the Residency Director.
3. Specific Additional Qualifications required for the PA I position.
a. NCCPA certified
b. License as a Physician Assistant in Massachusetts
c. Maintain current DEA and Massachusetts Controlled Substance Licenses
d. Complete 100 hours of CME according to national and state regulations during each 2 year period of employment.
e. Maintain current BLS/ACLS certification
f. Complete BWH Professionalism Program within 3 months of date of hire.
g. Complete BWH Procedural Sedation Training if determined necessary.
SKILLS ABILITIES COMPETENCIES REQUIRED:
Proficient with BWH computer systems as needed to independently perform daily tasks such as order entry documentation and note entry/ lab tests/results/electronic prescriptions incident reporting inbasket communication all necessary Epic/eCare modules(including but not limited to accessing radiology studies/navigation within studies/comparison viewing/utilization of tools and utilities for studies/retrieving preliminary reports).
Consistently provide high quality and timely documentation including admission notes progress notes discharge summaries and dictations. Thorough instruction for continuing care and followup should part of all discharge summaries and dictations.
Must be energetic quality driven and have proven ability to be a productive member of a cohesive team of peers dedicated to providing optimum patient care.
Must exercise the highest degree of professional judgment under the direct supervision and guidance of a licensed physician.
Must maintain a professional attitude towards patients and exercise such mature independent originality in solving problems not covered by existing guidelines within legal limits to meet the needs of the patient.
Must maintain a professional attitude towards patients at all times and an excellent and collegial working relationship with other personnel involved in patient care.
Must demonstrate understanding of procedures policies and documentation required to ensure compliance with hospital standards of care.
For the most common diagnoses admitted to the Otolaryngology Service the PA I will demonstrate competency to effectively diagnose formulate appropriate treatment plan and efficiently implement the plan without much direction and with an increasing level of independence from the attending physician.
Proficient knowledge in or ability to learn straight and flexible laryngoscopy and nasal endoscopy nasal cautery tracheoscopy removal of foreign bodies and ear blockage under microscope and other procedural skills after demonstrating competencies and evidence of performance and maintaining privileges in these areas.
Clinical skills and knowledge as well as professionalism of the PA I candidate are assessed by the supervising Otolaryngology Attending under the direction of the Otolaryngology Residency Director.
If a candidate does not pass the competency assessment the Otolaryngology Attending MDs will provide feedback on additional development needs. Reassessment is possible after 6 months of focused development.
Competencies are assessed by Otolaryngology Attendings either through direct observation or verbal communication. The attending physician(s) performing the assessment will fill out a competency assessment and the PAI is responsible for providing example cases which he or she has cared for patients with the most common diagnosis for the attending to review.
This is a per diem position.
