Job Description

QUALIFICATIONS:
1.Must have an advanced degree or a Doctorate of Pharmacy from an accredited college of pharmacy or be currently enrolled in an advanced degree program. Must be licensed to practice in the Commonwealth of Massachusetts. Equivalent years of professional practice experience will also be considered. 2.Postgraduate residency fellowship or Board Certified Pharmacotherapy Specialist is highly desirable.
3.Must complete 15 continuing education credits per year to maintain licensure.
4.Must have 3 years experience at the senior pharmacist level with performance consistently at the highly effective level or above while demonstrating required competency based on work experience and/or postgraduate training.
5. Upon department orientation and annually thereafter must complete:
Safety training:
OSHA/Fire safety
Infection control
Safe handling of chemotherapeutic agents

Competency assessments in areas of:
Agespecific pharmacotherapy
Processspecific skills
Clinical skills
Populationspecific pharmacotherapy
Aseptic technique

SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:
1.Must have excellent oral and interpersonal communication skills to communicate effectively with members of other healthcare disciplines patients and pharmacy staff.
2.Must be able to plan and coordinate the activities of pharmacy staff in accordance with Policy and Procedures.
3.Must be able to properly motivate and supervise support personnel.
4.Priorities and organizes work while under stress and with time constraints. Must pay attention to detail virtually 100 of work time and possess critical thinking and decision making skills.
5.Must be able to set and change priorities and accept additional responsibility as needed.
6. Must be able to respond positively to new approaches and changes in the work environment.
7.Must have extensive knowledge and ability in all areas of the department including: IV Admixtures Drug Information and Drug Distribution.
8.Must be able to train new employees.
9.Possess enhanced knowledge of the principles of growth and development over the life span of the neonate adolescent adult and geriatric patient. In addition the individual must possess the ability to interpret patient data relative to age specific needs and provide care as described in the departments policies/procedures.
10.Must have comprehensive knowledge of pharmacotherapy.
11.Ability to work collaboratively as a member of a multidisciplinary patient care team.

SUPERVISORY RESPONSIBILITY:
1.When required will supervise the activities of all supportive personnel assigned to a shift.
2.When required will supervise and direct the activities of all staff pharmacists assigned to a shift.
3.Directly responsible for the coordination of pharmacy services for an assigned patient care area. .
GENERAL SUMMARY/ OVERVIEW STATEMENT:
Responsible for reviewing and assessing the quality of pharmacotherapy for patients in assigned coverage areas. Responsible for the optimization of drug therapy by identification resolution and prevention of drug related problems and improving therapeutic outcomes.
Also responsible for overseeing the preparation compounding and dispensing of medications to patients in accordance with the laws and regulations governing pharmacists and the practice of pharmacy by the appropriate state and federal agencies.
Work independently without direct supervision within the limits of established pharmacy practice and within the scope of departmental policies and procedures as a member of the patient care team. Coordinates the provision of pharmacy services to assigned coverage areas. Provides leadership and guidance to staff pharmacists senior pharmacists and support personnel assigned to their shift.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
1.Assures the accurate and timely delivery of pharmacy services on their assigned shift.
2.Oversees the preparation compounding and dispensing of medications to patients in accordance with the laws and regulations of the Board of Registration in Pharmacy Department of Public Health Food and Drug Administration and other regulatory agencies.
3.Responsible for interpreting and implementing all departmental policies and procedures on the assigned shift.
4.Interprets clarifies and evaluates all medication orders for completeness and appropriateness. Communicates with physicians and nurses on medication order discrepancies possible drug interactions or stability problems. Assures that patient medication profiles contain all appropriate medication information and clinical data.
5.Provides prospective evaluation and monitoring of patient drug therapy. Recommends appropriate medication doses and adjustments as indicated by patient conditions. Identifies documents resolves and works to prevent drug therapy related problems.
6.Documents and reports all clinical interventions adverse drug events and drug therapy profile reviews. Demonstrates competency by documenting a minimum of 25 ADRs and 400 clinical interventions annually.
7.Provides drug information and pharmacotherapeutic consultations to physicians nurses and other healthcare professionals regarding appropriate drug therapy. Provides expert advice on drug interactions side effects incompatibilities allergies and drug administration.
8.When required supervises and verifies the accuracy of all work completed by supportive personnel assigned to the area.
9.Responsible for submitting required reports in assigned areas without direct supervision in a timely fashion.
10.Attends and participates in medical and multidisciplinary rounds on a daily basis. Participates in specialty rounds as needed and as time allows.
11.Provides discharge medication and medication history counseling to patients when appropriate.
12.Maintains skill and competency levels to comply with departmental and hospital Quality Assurance standards.
13.Assists with and provides feedback to pharmacy managers for the annual performance appraisals of staff pharmacists and senior pharmacists.
14.Actively participates in the development and performance of drug use educational materials and other drug information tools and monitoring initiatives. Actively participates in keeping the Drug Administration Guideline updated.
15.Medication management initiatives are part of pharmacist activities. Demonstrates this by creating an original cost savings initiative (includes presentation to involved teams and documentation of implementation and presentation of results). Also serves as a member of a Care Improvement Team.
16.Monitors drug therapy to prevent contraindications duplicate therapy drug interactions and allergic reactions. Reports monthly adverse drug reaction summary data to pharmacy leadership and appropriate hospital committees (Drug Safety).
17.Takes the lead role with the training and development of new professional staff.
18.Attends participates in and conducts continuing education programs and conferences to keep current with developments in the profession and to maintain licensure. Provides comprehensive CE programs for the department and presents a minimum of 2 CE programs to the staff annually. Attends participates in and conducts journal club review inservices and case studies for afternoon pharmacist meetings. Required to obtain 15 CEUs annually.
19. Maintains pharmaceutical care expertise through membership and participation in professional organizations or societies; demonstrates this by submission of 1 poster/abstract per year to AACP or ASHP. Advanced practice certification is highly desirable.
20. Understands the importance of additional education. Demonstrates this by possessing an advanced degree or by being enrolled in an advanced degree program with greater than 50 of the program completed.
21.Publish a minimum of 1 clinical article per year in a peerreviewed medical or pharmaceutical journal. Contributes a minimum of 1 article in the Pharmacy and Therapeutics Perspectives Newsletter.
22. Contributes to the development of peers and the achievement of departmental strategic plans.Demonstrates this by participation in the development of one competency exam per year in an area of expertise.
23.Serves as a preceptor for clinical clerkship students and pharmacy externs as outlined by the Massachusetts Board of Registration in Pharmacy and the local colleges of pharmacy. Demonstrates this by providing documentation of the preceptorship of 3 clinical clerkship students per year.
24.Participates in department quality improvement efforts.
25.Demonstrates advanced problem identification and resolution abilities and advanced therapeutic knowledge; demonstrates this by performing one drug use evaluation (DUE) presenting the results to the DUE committee. Participates in the development and implementation of corrective plans if necessary.
26.Obtains certificate in pharmacokinetic dosing and monitoring of selected medications. Provides selective consultations and recommends dose adjustments and laboratory monitoring as required to meet patient needs.
27.Must be credentialled to provide enhanced patient care services ie. blood pressure monitoring glucometry peak flow meters and ACLS.
28. Performs all other responsibilities as assigned.
