Job Description

QUALIFICATIONS:

Licenses/Certifications: Current LPN Licensure in the Commonwealth of Massachusetts.

Education: Graduate of an accredited Licensed Practical Nursing Program.

Experience: Some experience in a healthcare setting preferred.

SKILLS/ABILITIES/COMPETENCIES REQUIRED:

Technical Skills/Competencies Required:

Clinical Skills:

Knowledge of practice operations and standards.

Intermediate/advanced knowledge and use of medical terminology.

Basic knowledge of patient education: interviewing and listening skills teaching and engagement techniques understanding of healthrelated conditions and information.

Ability to accurately perform vital signs and obtain pertinent health information including but not limited to; temperature pulse respirations blood pressure height and weight.

Ability to administer medication; routes of administration may include injections intravenous and oral medications as per BWH Nursing Policy Med01.

Ability to accurately perform basic math calculations as necessary to engage in safe medication administration.

Ability to participate in the medication reconciliation processes and related prescription management tasks including prior authorizations.

Knowledge and ability to perform specific procedures and tests as appropriate to the clinical setting.

Communication skills: Demonstrates effective oral and written communication skills. Communicates in a professional courteous clear and concise manner. Facilitates therapeutic relationships between patients families and other members of the health care team.

Writing Skills: Ability to correspond and communicate with others clearly and effectively in writing (via email memo or interoffice note documentation of patient information) and to take complete and accurate messages.

Phone skills: Ability to telephonically support the practice by answering and screening calls offering information as appropriate and facilitating communication.

Organization Skills: Ability to manage work processes in a neat and orderly way. Ability to manage multiple tasks effectively follow established protocols and work within systems.

System Skills: Demonstrates ability to utilize technology in the clinical setting. Basic computer skills. Ability to type accurately and enter data at an intermediate level. Ability to become proficient in LMR/BICS/IDX and other appropriate applications. Ability to utilize handheld technology and download related data to the electronic health record.

Addendum A

Serve as a key member of the Primary Care Team within the Fish Center for Womens Health

Serve as LPN member of one of three patient care teams within Primary Care

Participate in quality improvement and other team projects

Responsible for following up on patients who have been discharged from an emergency room or inpatient visit to ensure continuity of care

Manage prescription refill process for patient care team and assist with managing refills for other teams as necessary

Provide cross coverage to other Primary Care and/or Specialties/Gynecology/Dermatology teams within the Fish Center as necessary

Ability to actively listen to patients and understand their needs

Ability to proactively trouble patient requests to help team providers respond to patients in a timely and efficient manner

Ability to effectively communicate with physicians coworkers and patients

Ability to work in a fastpaced environment

Ability to prioritize work and multitask without losing the ability to accurately complete work

Ability to solve problems

Ability to function effectively in a teambased environment

HOSPITAL WIDE RESPONSIBILITY:

Works within legal regulatory accreditation and ethical practice standards relevant to the position and as established by BWH/Partners; follows safe practices required for the position; complies with appropriate BWH and Partners policies and procedures; fulfills any training required by BWH and/or Partners as appropriate; brings potential matters of noncompliance to the attention of the supervisor or other appropriate hospital staff.

GENERAL SUMMARY/OVERVIEW STATEMENT:

The Licensed Practical Nurse is an integral member of the healthcare team and is accountable for patient care as assigned by a licensed independent provider (LIP: MD NP PA) or a Registered Nurse (RN). The LPN participates in direct and indirect nursing care health maintenance patient instruction collaborative planning and rehabilitation as within scope of practice and consistent with his/her education and training. Developing relationships with patients families and colleagues the LPN functions as an interdisciplinary team member who provides quality patient care and contributes toward the attainment of important goals for the patient the practice and the organization.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Clinical Duties and Responsibilities:

Demonstrates true engagement and enthusiasm to provide nursing care for patients and families.

Promotes an inclusive environment supportive of patient and family centered care for a diverse community.

Accepts assignments for patient care within the scope of his/her preparation and within current standards of nursing practice. In conjunction with a Licensed Independent Provider (LIP) or RN participates in the assessment planning and evaluation of nursing care delivered.

Collaborates as a member of the health care team contributing toward a supportive and therapeutic environment that promotes the attainment of patient goals that are specific measurable actionoriented realistic and timely.

Accurately obtains vital signs elicits information about the patients basic health status administers relevant nursing care within scope of practice and as directed by the LIP/RN and documents as appropriate in the electronic health record.

Administers medication as ordered by the LIP and according to established policies and procedures.

Performs routine tests that may include but are not limited to: electrocardiograms vision and hearing screening and the collection of specimens for laboratory testing and point of care.

Participates in teambased care and patient education initiatives to support the health and wellness of patients in conjunction with the LIP/RN. Offers basic instruction per protocol or as directed. Reinforces patient instruction as provided by LIP/RN.

Participates in organizational and unitbased quality improvement initiatives and compliance readiness.

Participates in telephone screening of patient calls; may provide instruction to patient when following approved protocol or as directed by the LIP/RN.

Participates in prescription management tasks including preparation of new prescriptions and renewals requiring LIP approval prior authorizations and medication reconciliation.

Appropriately assigns patient care to others within the scope of his/her practice and maintains responsibility for the optimal outcomes of such care.

Demonstrates a commitment to learning maintaining and developing additional competencies through a variety of methods that may include but not be limited to direct patient care participation in Center of Nursing Excellence and unitbased learning opportunities reading journals and maintaining currency in practice.

Clinic Operations:

Performs a variety of tasks including those that may be facilitated by Medical Assistants and administrative staff which have an effect on the overall operations of the unit. These include flow stocking of rooms scheduling tasks and all other duties as assigned.

Appropriately utilizes organizational resources including management of time supplies equipment and other assets.
