Job Description

The Spaulding Rehabilitation Network is seeking an experienced per diem Physical Therapist for ourWest RoxburySkilled Nursing Facility (SNF) to provide physical therapy services to our residents. Must be a graduate from an accredited Physical Therapist Program; current MA State license in Physical Therapy required clinical experience required. Provide each patient with a complete age appropriate evaluation and accurately interpret evaluation findings. The evaluation should reflect patient/resident history relevant systems review and appropriate tests and measures. The evaluation should identify all pertinent patient problems and present resources with consideration for patients age goals cultural and religious background. In addition the evaluation should predict the expected outcome and reflect realistic objective goals for patient performance. Develop and execute a treatment program which reflects an understanding of patients age and situation including discharge plan. The program reflects input from patient family and team members. Select treatment procedures that efficiently address the patients needs with consideration given to system and home constraints. Treatment programs utilize appropriate physical therapy procedures and include ongoing patient and family education. Execute home visits when indicated. Ensure procurement of equipment and services prior to discharge. Regularly reassess patient status and modify and revise goals plan or treatments as appropriate. Document all patient information within established time lines thoroughly accurately and on appropriate forms. Provide timely written information to other health care providers insurance representatives or equipment vendors. Accurately document administrative information for the department. Orient train and mentor the following persons: department orientees PTAs support personnel other health professionals student affiliates or interns. Completes tasks in a timely manner. Is punctual and attends required meetings including staff meetings team meetings and committee meetings. Meet or exceed established level of productivity per day. Manage equipment and space efficiently and effectively. Participates in program development and quality improvement activities as requested. Exhibits courtesy compassion and respect to patients families visitors physicians and coworkers and adheres to established customer service standards.

Graduate of an accredited Physical Therapist program Licensed (and in good standing) as a physical therapist in the Commonwealth of Massachusetts. Safely execute all job responsibilities without risking injury to self or patients. Demonstrates the ability to assess and interact with patients and families using the theories of human growth and development family systems and cultural background. Basic Skills / Abilities Required: Must demonstrate effective verbal and written communication skills with the English language. Must demonstrate basic skills in math and computer science. Must demonstrate appropriate knowledge of physical therapy examination evaluation and treatment techniques to manage the rehabilitation population at SRH. Must demonstrate the ability to perform modify and record all physical therapy skills with relevance to appropriate thoroughness and sequencing. Must demonstrate effective problemsolving skills for physical therapy evaluation interpretation treatment planning and taking into consideration patients age psychosocial cultural and religious background. Must demonstrate basic skills in teaching others and selfassessment of learning needs strengths and professional goals. May maintain variable work schedule depending on hospital/program needs to provide patient care (evening holidays weekends and travel). For the Physical Therapist the working condition is primarily defined by the patient environment. 1. Delivery of physical therapy services takes place in any/or all common areas and grounds and the patient care units including but not limited to the patient(s) bed room unit hallways unit therapy gym bathroom and kitchen. Regulation of environmental temperature cleanliness and materials storage is defined by hospital standards for all employees. 2. Candidate is expected to perform patient handling and transfers of various sized and mobility levels with assistance of mechanical aids other personnel or alone as appropriate to patient variables. 4. Candidate must demonstrate motor coordination and dexterity for implementation of therapeutic techniques manipulation of therapy materials and documentation. 5. Candidate is exposed to psychological stress of interaction with individuals who may have psychological and emotional deficits and medical instability. 6. There is potential exposure to blood blood products and body fluids.
