Job Description

Clinical Practice
o Has a sound knowledge of nursing medical and scientific principles and appropriately applies those principles to practice.
o Performs and records the patient history and physical exam and discusses findings with the physician.
    • Utilizes advanced nursing knowledge to identify prevent or solve complex and/or recurring patient care problems.
    • Follows and manages patients (in collaboration with MD) enhancing continuity of care.
    • Performs (oversees/assures completion of) specialized procedures particular to the select population mutually agreed upon with collaborating physician.
    • Performs medically delegated aspects of patient care as agreed upon by supervising physician in according to practice guidelines.
    • Collaborates with unit staff and physicians to facilitate a comprehensive plan of care and coordinate resources.
    • Writes orders which are in accord with current guidelines.
    • Orders interprets and evaluates appropriate lab and diagnostic tests.
    • Documents plan of care in patients medical record and PEPL notes.
o Maintains and practices in accord with practitioner guidelines per state and hospital regulations.
o Maintains patients right to privacy; assuring patient dignity in the provision of care.
Coordinating VAD and ECMO care:
  • Teaming with Cardiologists Cardiac Surgeons Transplant Coordinators Outpatient and Inpatient Coordinators and support staff while performing MCS coordinating care activity for all patients at MGH who are being treated with or being evaluated for MCS treatment options.
    • Serves as the central coordinating person for accepting and triaging MCS referrals from referring physicians Outpatient Access Coordinators inpatient transfers or selfreferred patients who do not have an existing relationship with an MGH cardiologist or cardiac surgeon.
    • Coordinates multidisciplinary MCS patient teams that can include but are not limited to physicians nurses social workers pharmacists physical therapists case managers perfusionists palliative care staff and VAD equipment technicians.
    • Coordinates MCS patient care and education as well as visits back to the hospital for followup care when patients are at designated rehabilitation facilities.
    • Maintains continuous communication with community agencies regarding ongoing care and support.
    • Maintains all MCS related information that supports management of the MCS program.
    • Communicates anticipated patient needs to appropriate physicians nurses and admitting departments.
    • Participates in cell phone and beeper contact for urgent problem solving during on and off hours.
    • Identifies and resolves service delivery problems and recommends methods to improve and streamline services.

Liaison with VAD vendors

  • Works in concert with the MGH Cardiac Procedure Services and vendors
    • Communicates on a timely basis the status of all VADs
    • Reports equipment related problems and seeks resolution
VAD Outreach/Education/Promotion
In collaboration with Medical and Surgical Directors of VAD program is responsible for developing and implementing an outreach initiative for patients community public agencies and including referring physician family and clinician education.
    • Provide VAD education to patient and their family members.
    • Followup and continuously communicate with referring and primary care physicians inside and outside of MGH.
    • Meets with community services and provides education about VAD care in the event of an emergency. Agencies include:
      • Police and fire departments
      • Local hospitals
      • Local EMTs
      • Home health agencies
      • Local utility companies
      • Designated rehabilitation facilities
    • Attends appropriate cardiacrelated events to meet new potential referring physicians.
    • Meets with existing and potential referring physicians to communicate and promote MGH VAD program.
    • Develops and provides patient and community education materials related to VADs (with assistance from MGH Marketing).

VAD Joint Commission Disease Specific Certification

Working with Sr. Administrative Director MGH Compliance and MGH Center for Quality and Safety the VAD coordinator will be responsible for:
Maintaining and fostering JC standards for VADs
Coordinating JC site visits and assuring MGH meets the JC standards in preparation for the biannual visit.
Coordinating Quality Assurance and Performance Improvement initiatives as required by the JC and reports to the JC as required.
Responsible for assisting with midcycle reporting to the JC
Assisting each member of the expanded VAD team with their role related to certification requirements.
Maintaining and updating Clinical Practice Guidelines and educating other providers as needed
Assisting with the management of Sharepoint site for VAD resources
Coordinating with VAD multidisciplinary teams that include all of the providers who participate in the management and care of the VAD patients.
Maintain VAD patient related information that is required by program
Viewed as an expert in VAD education monitoring and reporting program quality metrics and other requirements/regulations to support the JC VAD regulations.
Quality Assurance Process Improvement
  • Meets routinely with MCS Physician Directors to report on patient activities problems and recommendations.
  • Evaluates program effectiveness/performance by comparing actual outcomes to expectations.
Monitors referrals to and from MCS Program and its outcomes to meet performance benchmarks.
Assures that quality and performance improvement standards are developed implemented and maintained
  • Participates as requested in MGH and Partners quality initiatives.
  • Participates in various hospital and system committees as assigned.
  • With support from IS and PHS Specialty Services Development develops and maintains VAD and MCS patient tracking system.
  • Interacts and coordinates with Cardiac Access Coordinator on a regular basis.
  • Provides oversight to the Intermacs registry
  • Provides coordination and oversight for MCS research

  • Adheres to the professions clinical standards and Code of Ethics and upholds practice and programmatic standards that may include:
    • Assures informed consent for evaluation care processes and procedures as appropriate
    • Assures access for patients to treatment alternatives
    • Protects the rights of patients their families/legal guardians and health care team members
    • Respects individual privacy and holds confidential all information obtained in the course of practice
    • Assures open and clear communication with patients families and health care team members without bias or discrimination
    • Maintains cooperative relationships with other health care professionals
    • Acts to protect the public trust when health care and safety are endangered
    • Assists other healthcare facilities and community organizations with educational presentations and information appropriate to the needs of MCS program
    • Reviews evaluates and revises education materials and programs for MCS related practice
    • Identifies pertinent nursing and health delivery problems for investigation.
  • Adheres to the Department and Hospital standards policies and procedures.
  • Promotes Departmental and Hospital public relations/risk management.
Professional Development:
  • Initiates participates and contributes to educational programs for staff at the unit/service/interdepartmental levels.
  • Reviews and integrates into practice the current professional literature and research findings for clinical specialty
  • Supports and participates in research efforts to advance knowledge and to promote researchbased practice.
  • Shares knowledge through mentoring/precepting newly hired coordinators transplant staff nursing staff and other staff as needed
  • Responsible for own professional growth and maintenance of required credentials. Adheres to requirements for mandatory continuing education and certification specific to the position and licensure (Completes the 100 hours of continuing medical education necessary during each 2year period of employment). Identifies personal learning needs and goals and develops a plan to meet them. Maintains and updates clinical knowledge and skills based on current health care practice/research.

Current licensure in the Commonwealth of Massachusetts as a registered nurse in the expanded role.
Masters Degree in Nursing.
National certification in advanced practice nursing area.
Minimum of 5 years of clinical nursing practice with background in Cardiac Care
Minimum of 23 years in the cardiothoracic specialty
Experience in VAD Joint Commission Disease Specific Certification
