Job Description
Primary Care Clinic at Tsehootsooi Medical Center.Title: Care Coordinator (case manager) nurse positions within our Primary Care clinic:Requirement: Three 3 year Case Management Experience.Shift: Weekdays Monday through Fridays from 8a to 5p with One 1 hour lunch no weekends or holidays. Collaborate with support nursing staff and with providers for patient care.Housing: Information can be found under the Facility Group Document section. Orientation: on Mondays only If your traveler misses the compliance deadline they will be pushed to the following scheduled Orientation.
Shift: MF 85/730430 no holidays no weekends. Maximum of 2 Days Call off (Unapproved RTO) during contract.
