Job Description
Envision Hospicewas established in 2005 by six healthcare professionals who wanted to make a difference in their community.
We are currently seeking a dynamicLicensed Practical Nurse (LPN or LVN) to provide exceptional care to patients who require onetoone attention and monitoring in their homes in the Kitsap County WA area including homes assisted living facilities and skilled nursing facilities.
We are offeringLPN opportunities that are fulltime parttime or PRN (per diem) and have flexible schedules depending on availability.

Essential Functions:
Participates in the oncall rotation to provide coverage as needed.
Client assessments Intervention and overall patient care fulfilling patient care plans.
Following physician orders and acting in compliance with the states Nurse Practice Act any applicable licensure/certification requirements and our policies and procedures.
Strong commitment to delivering exceptional care while demonstrating compassion and integrity.
Comprehensive and precise communication skills
Provide onetoone care with your patients in their homes assisted living facilities and longterm care.
Minimum Qualifications:
- Graduate of an accredited or stateapproved nursing school
- Licensed as an LPN in the State of Washington
- Enjoy a truly patientcentered focus.
- A minimum of 1 year of experience in an acute care setting; home care or hospice experiencepreferred.
