Pediatric Care Manager (LCSW)
A Pediatric Care Manager (LCSW) is needed in St. Louis MO.

This position is with a communitybased nonprofit organization dedicated to providing compassionate care to children and families through comprehensive social work and care management services.
Salary and Benefits- Salary Range: $65000 $90000
- Student Loan Repayment: Up to $110000

- Flexible Scheduling: 5day or 4day work week available
- Provide care management and psychosocial support to pediatric patients and their families
- Collaborate with a multidisciplinary team including pediatricians nurses therapists and psychologists
- Develop and implement individualized treatment and care coordination plans
- Conduct assessments interventions and referrals for social services
- Educate families on available resources and assist with navigating healthcare systems
- May supervise and mentor social work interns or earlycareer professionals as needed
- Compassionate and patientcentered approach
- Clear and Active LCSW License in the State of Missouri
- Masters Degree in Social Work (MSW) from an accredited program
- Experience in an outpatient pediatric or integrated care setting preferred
- Familiarity with EMR systems (EPIC preferred)
