Hours: 37.5 hours per week
Salary: $39.17 $48.96 per hour
Shifts: Day Shifts (Monday Friday)

As a valued member of TeamJDMI you will play a key role in supporting JDMI clinical operations as the Clinical Intake Coordinator plays a pivotal role in screening patients to determine eligibility for the OLSP (Ontario Lung Screening Program).

Duties

  • Utilize clinical knowledge/experience to serve as an entry point for the timely access to appropriate clinical assessment and treatment services
  • Obtain and maintain current knowledge of internal/external services to communicate timely information
  • Collaborate with representatives from all services to plan develop implement and evaluate a systems approach to client care
  • Identify gaps in service and participating in addressing such internally and externally
  • Play a collaborative role with respect to the management of resource utilization
  • Evaluate the effectiveness of the role to facilitate decisionmaking

Qualifications :

  • At minimum completion of a Bachelors degree in Health Sciences or recognized equivalent required
  • Practical and related experience in direct clinical care including intake functions/triaging as well as community relations and service development
  • Previous experience in patient counselling
  • Demonstrated ability to work independently and collaboratively as part of a multidisciplinary team
  • Client service oriented and ability to work with the target population
  • Demonstrated knowledge of clinical pathways and Continuous Quality Improvement (CQI)
  • Knowledge of smoking cessation counselling techniques is an asset
  • Ability to make quick assessment
  • Proven skills as a problem recognition and problem solving
  • Exercise initiative and good judgment with ability to multitask
  • Compliance with confidentiality requirements
  • Effective organizational interpersonal and communication skills
  • Knowledge of applicable legislative UHN and/or departmental policies
  • Ability to perform duties in a professional and courteous manner and produce high quality work while meeting deadlines in accordance to UHN standards
