PT Hybrid Integrated Behavioral Health Counselor
An Integrated Behavioral Health Counselor (Hybrid Remote) is needed in San Jose CA.

Salary and Benefits
- Competitive pay range of $35 $57 hourly
- 24 hours per week

Responsibilities
- Provide culturally sensitive clientcentered individual group and family counseling and rehabilitation services.
- Facilitate referrals to community resources and refer to case management services.
- Conduct psychosocial assessments to identify patients needs and formulate treatment plans.
- Document and maintain clinical records per appropriate standards and regulations.
- Participate in clinical supervision team meetings and training.
- Collaborate with the primary care team to integrate behavioral health into the overall patient care plan. Participate in case conferences and care coordination with other AACI programs.
- Completed and maintain LPS 5150 Authorization certification and provided officeroftheday support.
- Communicate well with people of diverse cultural professional and experiential backgrounds.
- Establish and maintain effective work relationships as part of a multidisciplinary team.
- Offer immediate support and crisis intervention for patients experiencing acute behavioral health issues.
- Complete other related duties and projects as assigned.
Requirements:
- ASW AMFT APCC LMFT LCSW or Licensed Psychologist in the state of California.
- Masters degree in social work or psychology.
- One year or more of clinical experience in Integrated Behavioral Health Specialty Mental Health and/or Brief Therapy Models.
- Experience in providing behavioral health counseling in a healthcare or similar setting is highly desired.
- Ability to perform the duties described above.
- Knowledge of laws and ethical standards about clinical care.
- Knowledge of DSMV criteria for diagnosing behavioral health illnesses.
- Experience with electronic health records and proficiency in documentation.
- Knowledge of Improving Mood Promoting Access to Collaborative Treatment (IMPACT) Model of Care and Managed Care Models preferred.
- Verbal and written fluency in English is required; Bilingual skills in a second language (particularly Spanish Mandarin and Vietnamese) are desired but not required.
