You will...

• Educate patients and their families on proper care techniques medication administration and health promotion strategies.
• Communicate effectively with patients families and healthcare providers to ensure continuity of care and positive outcomes.
• Participate in ongoing training and professional development opportunities to enhance your skills and knowledge.
• Provide oneonone skilled nursing care and treatment to patients in their home.
• Assist the registered nurse in assessment of patients condition and contribute information to aid in planning and evaluating care.
• Demonstrate knowledge of home health and competencies in disciplinespecific patient care skills required by the care center to provide patient care.
• Perform comprehensive patient assessments and collaborate with a multidisciplinary care team to develop an individualized plan of care as per physician orders that achieve desired outcomes and goals.
• Check patients vital signs Perform wound care and educate patients and their families about their medications and chronic conditions.
• Provide patient episode management per protocol.
• Complete documentation timely and accurately according to industry standards conventions and guidelines including subsequent visits or other required documentation.

You have...

• Current and unencumbered license as a Licensed Practical Nurse (LPN)
• Strong assessment critical thinking and communication skills.
• 1 year(s) of Certified Home Health experience as an LPN
• Ability to work independently and as part of a dynamic team.
• Comfortability using technical tools and software/EMR for visit documentation
• Current CPR certification.
• Reliable transportation current drivers license and car insurance coverage.
• Proof of Immunization/Exemption for Flu and COVID19.
• TB Test or Chest XRay.
