Shifts: Evening Shift 3:00 PM 11:00 PM)
Job Type: Fulltime Contract 40 hours per week)
Location: Pontiac IL
- Deliver direct patient care under the supervision of registered nurses or physicians
- Collect and document vital signs and patient health history
- Administer medications and treatments while ensuring accurate documentation
- Provide patient care in a correctional facility setting
- Assist with specimen collection and maintain laboratory standards
- Collaborate with healthcare teams to develop and implement patient care plans
- Educate patients on health management and preventive care
- Participate in case management activities to ensure comprehensive patient support
- Valid LPN license in the state of practice
- Minimum 1 year of LPN experience preferred
- Strong skills in medical documentation and patient assessment
- Ability to work effectively in a teamoriented environment while providing compassionate care
- Excellent communication skills both verbal and written
- Commitment to confidentiality and adherence to healthcare regulations
- BLS Certification
- TB Test Physical Exam and Required Vaccinations
- COVID Vaccination
- Background Check and Drug Screen
