Job Description

Licensed Practical Nurse (LPN)

Licensed Practial Nurse (LPN)
The Licensed Practical Nurse is responsible for providing training for medication administration medical oversight for assigned individuals and advocacy for each individuals best medical care throughout Indiana. The LPN supports LEL Vision Mission and Values.
Reports to:
Director of Quality
Registered Nurse (RN)
Position Title will:
  • Works closely with Direct Support Coaches and Direct Support Professionals to ensure training on medication administration health and risk assessments and highrisk plans are addressed by individuals served and their families
  • Responsible for any healthcare coordination services provided by LEL
  • Provide the appropriate followup training advocacy and corrective action planning as needed
  • Perform trend analysis on an ongoing basis (and as appropriate in consultation with other members of management and/or LELs legal counsel) to identity safety and/or quality concerns
  • Report on the status of all ongoing medication administration high risk or health concerns of individuals served
  • Document services of individuals served timely and accurately as required by LEL policies and procedures.
  • Support healthcare coordination to all LEL Individuals served including Supported Living and Family Living including visits to each home to provide an individual physical assessment risk plans review medication administration records and evaluate health care issues as per Waiver Service Standards
  • Advocate for increasing Individual competencies and independence through best personcentered practices and Social Role Valorization theory application
  • Promote LifeCourse and life visioning to assure positive life trajectories as identified by the family and supporting team
  • CPR Instructor Certified or ability to become
  • CNA DSP HHA training courses
  • Demonstrates competencies as a Direct Support Professional by providing assistance to Individuals as they pursue the Good Life.
  • Other duties as assigned
Qualifications:
  • Must have a high school degree postsecondary degree preferred
  • Must hold a valid current license issued by the State of Indiana
  • Possess the knowledge of nursing principles methods and procedures
  • Possess the knowledge of techniques theory and practices of professional nursing
  • In home health care experience preferred
  • Must have experience in training and/or instructing others
  • The ability to plan organize and direct a variety of nursing activities
  • The ability to plan and supervise the work of others
  • In home health care experience preferred
  • The ability to establish effective relationships with LEL Individuals families Case Managers and coworkers
  • Must support LEL Individuals throughout the state
  • The ability to handle sensitive confidential information
  • Strong verbal and written communication skills
  • Comfortable using Microsoft Office
  • The ability to multitask and meet deadlines
  • Positive and pleasant attitude
Physical Requirements:
The employee must:

A. Regularly:

1.Speak;

2.Hear (both in person and using a telephone);

3.Sit;

4.Use hands to manipulate handle or feel;

5.Reach with hands and arms and

6.Lift and/or move up to five 5 pounds.

B. Frequently:

1.Stand walk stoop or kneel and

2.Lift and/or move up to ten 10 pounds.

C. Occasionally:

1.Operate a noncommercial automobile for distances up to 500 miles;

2.Climb and/or straddle and

3.Lift and/or move up to twenty 20 pounds.

While performing the duties of this job the employee is regularly exposed to an office environment in which the noise level is usually low to moderate. The employee is occasionally exposed to outside weather conditions as well as Individual residences in which the noise levels are usually moderate.
