As a valued member of our collaborative and supportive team you will work together to transform the lives of individuals by providing a comprehensive range of services that treat the whole person and empower patients to find their way forward.

What Youll Do

The InHome Therapy Services (IHTS) Qualified Professional provides case management and care coordination to support individuals in various environments including homes schools shelters detention centers and other community settings. Working alongside the IHTS Therapist the QP reinforces therapeutic interventions helps manage psychiatric and addiction symptoms and delivers crisis intervention to reduce future risks. Responsibilities include linking clients to vital community resources providing life skills training supporting caregivers in implementing homebased strategies and coordinating rehabilitative services to prevent outofhome placements. Additionally the QP may assist in crisis response peer reviews and new employee training while ensuring compliance with agency and service model requirements.

Hours will range from MondayFriday must be available for a flexible schedule that may require afternoon and evening sessions some possible weekend sessions. 24/7/365 on call rotation every 3 weeks.

What Were Looking For:

To join our team you must have passion for working with youth and families ability to enjoy a flexible schedule and do what it takes to meet families where they are at enjoy driving creative and innovative in order to meet children and familys needs work independently and be a team player.

We also require the following:

• Bachelors degree in human service field with two years post degree experience with population served (at risk youth); or
• Masters degree in human service field with one year post degree experience with population served (at risk youth). Must have documented experience with the population served
• Must be able to operate a motor vehicle and have a valid insurance and an unrestricted drivers license
