The Home Care/ Hospice (outpatient) Graduate Nurse applicant should have foundational interpersonal and communication skills a desire to work with a team and culture that empowers best practice patientcentered care and selfcare and a strong desire to pursue professional growth and leadership in nursing.
The foundation of the Home Care/ Hospice (outpatient) Nurse Residency program is to provide insight and development opportunities intended to optimize a Nurses experience for the first year. The Nurse Residency creates a positive foundation for the rest of their nursing career supporting the effectiveness of transition into practice. The Graduate Nurse will have a 1:1 preceptor for 12 weeks or more for specialty areas with monthly seminars for 12 months applying nursing science to a diverse population developing autonomy & critical thinking strategies and involvement in the discovery and implementation of evidencebased practice. The program focuses on professionalism leadership patientcentered care patient outcomes organizational enculturation nursing development and selfcare.
The Home Care/ Hospice (outpatient) Graduate Nurses will work together as a team to uncover best practices and research to enhance existing nursing practices within Bristol Hospital and present outcomes year end. The Graduate Nurse will participate with other nurses on shared governance committees to support the nursing structure of Bristol Hospital.
The Graduate Nurse is responsible for learning and practicing the delivery of coordinated nursing care for an assigned group of patients including nursing assessment planning implementation evaluation and transition of care.
The Graduate Nurse is responsible for carrying out routine nursing care tasks and using professional judgment and professional behavior in caring for our patient population. The Graduate Nurse will have the responsibility to supervise tasks of unlicensed personnel and to coordinate patient care.
The Graduate Nurse will learn the value of teamwork and be empowered to grow and learn in their Nurse Residency.
BSN preferred; Associates Degree accepted (with enrollment in BSN program 6 months after hire)
Official hire dependent on successfully passing NCLEX.
CT RN License required.
Active BLS through the American Heart Association or the American Red Cross
Valid CT Drivers License
