An Aging Care Manager 2 receives both formal and informal training and becomes knowledgeable of the rules and regulations of programs provided by the Agency. After training you will be able to perform intake and referral to services duties in addition to completing initial intake and document information from consumers. Your communication skills will be key as you will have the opportunity to be the initial contact person for consumers family members or professionals requesting information regarding services. This includes explaining and describing availability eligibility and program limitations or restrictions. You will maintain a caseload of consumers providing ongoing care management and coordination of services with other community resources as needed. Our ideal candidate must have the ability to manage multiple tasks and demonstrate close attention to detail which includes completion of paperwork work assignments and meeting deadlines. As abilities and understanding of available programs progress this professional will be assigned increased responsibility for arranging consumer services.
