Position Summary:
Provides counseling services including identifying screening and assessing clients to determine the need for substance use treatment. Provides referrals to communitybased organizations and coordinates care as needed. Responsible for facilitating therapeutic group and individual sessions. Responsible for completing and maintaining detailed OASAScompliant case records.
Direct Supervisor:Director of Behavioral Health Services
Duties and Responsibilities:
- Conduct intake interviews biopsychosocial mental health screenings.
- Formulate treatment plans and treatment plan reviews act as participants case manager to coordinate care within Crossroads and will external providers.
- Perform individual counseling duties which include holding weekly individual sessions with clients providing referrals to communitybased resources (healthcare social services and administrative e.g. help obtaining insurance) and developing client schedules.
- Initiate and maintain client records in accordance with policies and procedures.
- Ensure accurate and timely data entry of all client information into the agencys database; utilize electronic health record system.
- Facilitate assigned treatment groups.
- Perform drug testing as needed.
- Prepare monthly updates for court or other supervising agencies.
- Participate in treatment team case reviews.
- Consult with other professionals in support of clients treatment.
- Complete discharge documentation as needed.
Qualifications:
- LMSW or CASAC is required (CASAC is also preferred for social workers)
- Minimum of two years clinical/case management experience in working with clients that have substance use disorders is required.
- Experience working with the criminal justice population is preferred.
- Computer literacy (word processing email internet research data entry) is required.
- Ability to use an electronic health record system required.
- Bilingual (English/Spanish) preferred.
- Relevant personal experience in terms of recovery and/or justice involvement a plus.
