About Corewell Health South Hospice
We are a community based nonprofit organization serving 3 counties in Southwest Michigan: Cass Berrien and Van Buren for over 30 years. We specialize in providing end of life care and symptom management to individuals with a life expectancy of six month or less. We are one of the few Hospices in Southwest Michigan with a 16bed inpatient hospice residence to serve patients and families needing this environment. We serve individuals in their home nursing facilities and in the hospital.
Scope of Work
The RN is responsible for assessing planning implementing coordinating monitoring and evaluating the strategies and services required to meet the patients/familys endoflife care needs. An outcome based plan of care is created and continually adjusted through utilization of the nursing process relationship centered care principles and holistic philosophy assuring that the changing needs of the patient/family are met while facilitating a painfree peaceful sacred death. Exhibit Patients and Families First attitude. Responds with compassion flexibility and urgency to each patient need. Provides the care that patients want when they want it.
Qualifications
Required
Associates Degree Nursing
1 year of relevant experience preferably in hospice or related field
Registered Nurse (RN) State of Michigan required
Basic Life Support (BLS) through American Heart Association or American Red Cross within 90 Days required
Preferred
Bachelors Degree Nursing
Experience with dying patients and their families
