Job Description

- Complete initial and ongoing assessments to identify the physical psychosocial and environmental needs of home health patients.
- Assess patients conditions and chart observations and perform evaluation tasks including vital signs and medication review.
- Administer medication as prescribed by the patients physician.
- Provide effective communication to patients/clients their family members team members and other health care professionals.
- Treatments and dressings and recording according to physicians orders facility policy and safe nursing practice.
- Completion of precise and professional charting PCC according to PCC policy.
- Communication with residents and responsible parties regarding resident condition treatment appointments etc.
- Communication with pharmacy to assure proper medications for each resident.
- Coordination of all nursing functions of assigned unit/team.
- Hands on care of residents as needed and other duties as assigned.
- Graduate of an accredited Diploma Associate or Bachelors School of Nursing or vocational nursing
- Current state license as an RN
- Proof of current CPR certification
- Compassionate friendly demeanor
- Excellent communication skills
- Current drivers license and reliable transportation
- Ability to work a flexible schedule and travel locally
