- Provide oneonone skilled nursing care and treatment to patients in their homes.
- Assist registered nurses in assessing patients conditions and contributing to care planning.
- Demonstrate expertise in home health and disciplinespecific patient care skills.
- Perform comprehensive patient assessments and collaborate with multidisciplinary care teams.
- Monitor vital signs administer treatments and educate patients and families on medical conditions.
- Manage patient episodes according to protocol.
- Complete documentation accurately and timely.
You have...
- Current and unencumbered LPN license
- 1 year(s) of Certified Home Health experience as an LPN
- Current CPR certification
- Comfortable with technical tools and EMR for visit documentation
- Reliable transportation drivers license and car insurance
- Proof of Immunization/Exemption for Flu and COVID19
- TB Test or Chest XRay
