Job Description

QUALIFICATIONS:

1.Master of Physician Assistant Studies from anaccredited program approved by the AMA Board of Approval and Certification ofPA programs

2.Board Certification by the National Commission onCertification of Physician Assistants (NCCPA)

3.Commonwealth of Massachusetts PAlicense

  1. Additional license/certification/registration required to perform the job.
    1. Maintain current DEA and Massachusetts Controlled Substance Licenses
    2. Complete 100 hours of CME according to national and state regulations during each 2 year period of employment.
    3. Maintain current BLS/ACLS certification
    4. Complete BWH Professionalism Program within 3 months of date of hire

SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:

1. Strongpatientcare skills and the ability to learn and assist with evaluation andtreatment of outpatient cardiology patients.

2. Excellentorganizational skills with ability to work independently and prioritize dutiesappropriately.

3. Strongleadership and communication skills in order to work effectively with othermembers of the healthcare team including physicians nurses clinical staff andadministrative assistants.

4. Mustbe able to demonstrate the knowledge and skills necessary to provide care toadult patients ages 18 and above.


WORKING CONDITIONS:

Works extensively in our ambulatory clinic. This job mayrequire the exposure to potentially infectious material. The position requires the use of needles andother sharp instruments.

GENERAL SUMMARY/ OVERVIEW STATEMENT:

Cardiovascular Medicinebelieves in the team approach to medical care. In collaboration with a team of cardiologistthe Physician Assistant an expert in cardiovascular care will be involved inall aspects of ambulatory care in addition to maintaining an active independentpatient panel with the primary goal of ensuring the effective delivery of careof patients. The PA will have the leadership responsibility for the team andwill work closely with the Clinic Director and Administration.

PRINCIPAL DUTIES AND RESPONSIBILITIES:


A. Patient Care Organization

Performsin the role of Independent Licensed Provider (ILP) and is responsible to workboth independently and in collaboration with other providers to maintain apanel of patients. Primary area of responsibility is to develop and maintain anambulatory patient panel via ambulatory clinic sessions. (Example of ambulatoryclinic sessions: can be expected to maintain up to 8 sessions per week).

Assessesplans implements collaborates and evaluates the needs and care of patientswith acute and chronic diseases.

Promotesselfcare and collaborates with patients to establish realistic health goals.

Providespatient and family teaching staff education and consults and collaborateswith other health care providers; participates in nursing and medical research.

Manifests ahigh level of expertise in the assessment diagnosis and treatment of thecomplex responses of individuals families or communities to actual orpotential health problems related to their life limiting disease conditions.

Exhibits anexpert level of practice and breadth of knowledge in cardiovascular care.

Performsphysical examinations and provides preventive health measures appropriate topatient needs.

Ordersinterprets and evaluates appropriate lab and diagnostic tests to assesspatients clinical problems and health care needs.

Arrangesfor appropriate plan of care and followup based on outcome of diagnostic laband physical assessment findings. Seeksconsultation as appropriate.

Ordersmedications and writes prescriptions according to organizational and regulatorypolicies and procedures.

Recordspertinent patient care findings in the electronic medical record.

Setspriorities for appropriate and efficient management of patient care thatreflects evidence based practice and cost effective management of timeavailable human resources supplies and equipment.

Maintains an interdisciplinary approach toplanning and communicating patient care information by discussing patient datawith appropriate providers the clinical team and other health careprofessionals.

Incorporatesthe concepts of health maintenance prevention and promotion into dailypractice through patient education and counseling. Assists patients and families in selfcaremanagement through the provision of information tools and resources.

B. Leadership:

In collaboration with practice leadership develops andleads the clinical practice quality and safety of patient care of aninterdisciplinary team which may include registered nurses licensed practicalnurses medical assistants and others who are essential to the care deliverymodel.

Responsible for coordinating care and meeting patient care needs in conjunctionwith a variety of providers including; licensed independent providers acting inconsultation population managers care managers and others.

Identifiesopportunities for and initiates interdisciplinary collaboration to achieve teamand patient goals.

Utilizesavailable resources and methodologies to advance practice within the departmentand beyond with the aim of improving the care of all patients particularlythose most at risk for adverse outcomes.

Collaborateswith the other members of the team appreciating and valuing theircontributions and encouraging all to work at the top of his/her license/rolefor effective care and service to patients in the context of efficient use ofresources.

Establisheseffective interpersonal relationships.

Performsother related duties and responsibilities as required.

C. Quality Monitoring

Engages in regular performance improvement activities. Uses performance and patient outcome data forcontinuous quality improvement.Contributes to developing structure processes and systems to improvethe care and disease management of patients.

Collaborates with physicians PAs NPs and theinterdisciplinary team to refine team goals and objectives and further ensuresongoing continuous improvements to the care delivery model.

Analyzes interprets and presents team based care resultsto various audiences as appropriate including senior organizational leadersdepartmental and practice leaders nursing leadership colleagues and staff.

Translatesthemes uncovered through ongoing quality monitoring and evaluation into thecare model and the clinical practice of the interdisciplinary team.
