Division and Program Overview:
Health Promotion Services of RiverStone Health exists to improve the life health and safety of Yellowstone County residents at the population level. Services include the prevention of poor health outcomes and health behaviors through assessment rural school nursing health education community partnerships policy systems and environmental strategies.

The Pathways Hub Program Coordinator is responsible for the daily operations of the care coordination innovation component of the Yellowstone County crisis and diversion program. This position will work with community partners including area community health workers navigators care managers and like professions to establish a coordinated care model and regional hub.
Essential Functions/Major Duties and Responsibilities:
A. Program Coordinator Responsibilities 50
  • Coordinate efforts associated with the preparation of regulatory documents or submissions.
  • Review clinical protocols to ensure collection of data needed for regulatory submissions.
  • Participate in internal or external audits.
  • Compile and maintain regulatory documentation databases or systems.
  • Maintain current knowledge base of existing and emerging regulations standards or guidance documents.
  • Identify relevant guidance documents international standards or consensus standards.
  • Interpret regulatory rules or rule changes and ensure that they are communicated through policies and procedures.
  • Maintain relevant inventories of supplies.
  • Provide or coordinate services for guests patients staff as necessary.
  • Participates in quality improvement and Emergency Preparedness activities.
  • Advise clients or community groups on issues related to improving general health including substance misuse and resource navigation.
  • Advocate for individual or community health needs with government agencies or health service providers.
  • Attend community meetings or health fairs to understand community issues or build relationships with community members.
  • Maintain updated client records with plans notes appropriate forms or related information.
  • Teach classes or otherwise disseminate health information to school groups community groups or targeted families or individuals in a manner consistent with cultural norms.
  • Provide feedback to health and social service providers regarding improving service accessibility or acceptability.
  • Refer community members to needed health and social services.

B. Pathway Hub Coordinator Responsibilities 45
  • Lead the implementation of innovation project component of the crisis diversion contract ensuring adherence to timelines and milestones.
  • Collaborate with partners to improve coordination among care coordinators community health workers (CHWs) tenancy support specialists peer specialist navigators targeted case managers and similar professions.
  • Assist with continued development of the ASAP platform as an initial tool for systemic access to services to streamline access to services and ensure it aligns with the pathway hub model.
  • Utilize a collaborative process to establish standards of care coordination activities tools and referral best practices within Yellowstone County.
  • Evaluate and prioritize relevant funding mechanisms to support care coordinationlike positions in both clinical and communitybased settings.
  • Coordinate a feasibility study for establishing a pathways community hub model for Yellowstone County including investigation of partnerships policy and procedural needs funding mechanisms strategic activities and return on investment.
  • Monitor and record expenditures report on grant budget and assist in the development of budgets.
  • Responsible for all aspects of grant coordination including building and maintaining partnerships work plan development and implementation subawards trainings evaluation communications deadlines and required grant reports.
  • Develop and maintain partnerships with a diverse mix of local and state organizations and individuals to enhance positive project reach and impact in Yellowstone County.
  • Provide effective strategic and technical support to project partners.
  • Serve as a spokesperson/contact for the project and as the liaison to coordinate spokesperson(s) for community organizations newspapers other media and elected officials.
  • Serve as a liaison to the Community Health Needs Assessment work team to align project activities as appropriate.
  • Conduct and/or coordinate regular oral and written communication (including website and social media communication) about project goals facts policy advocacy and partner activities.
  • Regularly report to service internal and external partners progress on deliverables outcomes and metrics.
  • Produce maintain revise and submit required reporting documents in a timely manner with the Healthy By Design coordinator as final approval.
  • Coordinate and communicate with grantors regarding work being implemented and required reporting.
  • Perform effective public speaking to groups with varied interests and backgrounds; develop and deliver informative and persuasive communications to engage community partners to expand the reach of CHWs in Yellowstone County.
  • Support sustainability efforts for care coordinator community health worker and similar professions throughout the state of Montana.
  • Evaluate the return on investment for care coordinator community health worker and similar professions in partnership with supervisor and other relevant RiverStone Health staff.
  • Coordinate local training activities and skillbuilding opportunities.
  • Oversee crisis diversion support staff as appropriate.

Education and Experience:
Minimum Qualifications
  • High School Diploma
  • Equivalent of two 2 years experience in a directly related field such as health promotion nonprofit services health services or a directly related field or in experience in a position of similar or transferrable skills and duties
  • Any combination of experience and training which provide the equivalent scope of knowledge skills and abilities necessary to perform the work.
Preferred Qualifications:
  • Experience working with underserved population
  • Bachelors degree in related field
  • Experience working with community health workers care managers peer specialists navigators and/or related professions.

  • Up to 30 of travel time required for educational presentations. This includes overnight stays by vehicle and plane. Expected to drive a vehicle which requires sitting seeing and reading signs traffic signals other vehicles etc. Travel occurs in all weather conditions including extreme heat and cold.

Budget & Resource Management:
  • The position is responsible for managing resources including time educational materials and other resources necessary to deliver services. Decisions made by the position directly affect efficiency and cost of services.
This grant funded program is on a three year renewal cycle (June 30 2027.
