POSITION SUMMARY

The Job Coach II supports the CHS mission by assisting the Professional Managers with various services to ensure the department is providing outstanding supports and services to individuals. The Job Coach II can assist with case management duties that ultimately empower the individuals to achieve the highest level of independence through high quality and diverse services and supports.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Serve as a Role Model by exhibiting positive and professional behavior when supporting individuals with disabilities in the employment setting conducting job development services and interacting with funders individuals and the community.
  • Provide Assistance with Casework as directed by the Professional Manager (PM) by reporting client schedules and/or running Community Based Assessments drafting Standard Operating Procedures (SOPs) assistance with the Summer Work Experience (SWE) and drafting of Monthly Summaries/Monthly Progress Reports.
  • Conduct Site Visits to provide onsite feedback and conduct Quality Assurance measures ensuring that the job coaches and individuals understand the provisions of Home and Community Based Waivered Services (HCBS) and how the Employment Services Department can embody such practices.
  • Ensure Fluid Communication with Professional Manager to ensure that the individuals receiving services are in the appropriate authorizations any additional needs not expressed in case notes are known by the PM and all written communication is easy to understand professional and copied to the PM if not the intended recipient.
  • Demonstrate Best Practices for Integration of Individuals in their employment settings by assisting in developing natural supports coordination of transportation and appropriate workplace interactions with coworkers and patrons of said employer. The Job Coach II will ensure that the supports being provided to the individual are in alignment with the individuals teams expectations. Will ensure that all SOPs are up to date and supportive of the individuals progress at independent employment.
  • Ensure Regulatory Compliance by completing all required documentation to standard within the timeframes established by both the Center for Human Services as well as our respective funding sources. Additionally job coaches will actively participate in trainings and meetings demonstrating professionalism reliability satisfactory attendance and accountability. Cover job coaching shifts as needed to ensure coverage.
  • Strengthen Positive Work Culture by participating in effective courteous and professional communication; showing kindness and patience to individuals and coworkers demonstrating respect and responsibility in daily work and practicing advocacy and encouragement in all aspects of employment.

PHYSICAL REQUIREMENTS

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Lift carry squat bend move push or pull sit or stand repetitively and for long intermittent periods.
  • Prolonged periods sitting at a desk and working on a computer.
  • Must be able to lift 10 pounds at times and occasionally lift and/or move up to 25 pounds.
  • Specific vision abilities required by this job include close vision and ability to adjust focus while using the computer.

QUALIFICATIONS

REQUIRED EDUCATION

  • Associate degree (AA or AAS) plus one year of related work experience or any combination of education and experience to equal three years.

REQUIRED MISCELLANEOUS QUALIFICATIONS

  • Valid drivers license and valid vehicle insurance
  • Must have reliable form of communication
  • Ability to effectively communicate via written and verbal communication methods in a way that is understandable to all audiences.
  • Ability to satisfactorily complete mandatory (paid) trainings to include CPR/First Aid

PREFERRED EDUCATION and EXPERIENCE

  • Consistent work experience in the Employment Services Department at the Center for Human Services for a period of one year or greater (this can include historical experience).
