Job Description
Job Title: Adult Social Worker Front Door Team Stockport
Location: Stockport
Pay Rate: 30/per hour
A temporary position for an experienced Social Worker is available for a 3month contract. The role involves working with the Front Door Team and the Urgent Community Response Team. Key responsibilities include collaborating with Health colleagues to minimize hospital visits conducting visits as needed (including sameday responses) completing Care Act and shortterm assessments to promote independence at home and providing Social Work expertise within a multidisciplinary team. The position is officebased and requires a clean driving license and transport.
Qualifications and Experience:
To be successful in this role you must have Social Work England registration be eligible to work in the UK hold a qualification in Social Work at degree level and have postqualified experience.
