Job Summary

The Rehab Services Lead is responsible for the evaluation and administration of rehabilitative/habilitative techniques necessitated as a result of age trauma degenerative disease congenital condition or sudden onset of pathology which requires development maintenance or restoration of function. May be required to care for a variety of diagnostic groups including but not limited to the following: orthopedic and arthritis related conditions; neurological groups including CVA TBI degenerative neurological diseases SCI; and generalized weakness as a result of medical or surgical conditions. Rehab Services Lead is a qualified professional who directs supervises evaluates and provides therapy services to patients as prescribed by the attending Physician. The lead therapist helps supervise the therapy team and assists Management in the daytoday operations.

Essential Functions

  • Assessment:
    • Administers appropriate informal and/or formal evaluation procedures for all patient populations served.
    • Understands referral procedure and gathers pertinent data including verification of physician order
    • Determines appropriate assessment based on diagnosis age severity
    • Collects objective information related to patients functional status
    • Adequately interprets data collected
  • Planning:
    • Develops treatment plan goals and objectives based on analysis of the assessment and professional observations.
    • Identifies and prioritizes functional limitations based on assessment findings
    • Identifies realistic longterm and shortterm goals described in functional or behavioral terms appropriate to the individuals needs personal goals and expected outcomes
    • Determines treatment techniques and activities needed to achieve goals
    • Outlines the proposed intervention including frequency and duration
    • Achieves an interdisciplinary team approach by consulting with team members including patient/family regarding treatment plan and delivery model
    • Initiates discharge planning and recommendations for additional services to meet the needs of the patient
  • Treatment:
    • Utilizes appropriate treatment methods based on functional needs of patient.
    • Provided under the direct care of or under the direction/supervision of a therapist
    • Adheres to appropriate and safe application of modalities therapeutic exercises and activities
    • Is altered in accordance with changes in response or status
    • Is provided at a level that is consistent with current therapy practice
    • Is interdisciplinary when necessary to meet the needs of the patient
    • Discontinued when goals/outcomes achieved no longer a benefit from therapy patient declines to continue intervention
  • Education:
    • Utilizes appropriate education and training to provide knowledge of illness and treatment needs as well as skills and behaviors that promote recovery and improve function.
    • Is appropriate to the individuals age/developmental level gender education culture and socioeconomic status
    • Is presented in terminology understandable to patient/family
    • Documents patient understanding of the information issued
    • Utilizes various methods to achieve desired educational outcome
  • Documentation:
    • Completes appropriate documentation and communication of all aspects of patient care including results of initial evaluation plan of care interventions response to interventions changes in status reassessments education and discharge.
    • Is completed within designated time frames
    • Includes all relevant information as outlined by organizational/division policy accrediting agencies regulatory agencies and state practice act
    • Is clear concise and relevant to the type of care/treatment being provided
    • Is dated and appropriately authenticated by the therapist
    • Adheres to all laws and regulations regarding patient confidentiality and disclosure of protected health information
  • Professionalism:
    • Utilizes sound professional judgment based on knowledge skill education training and experience.
    • Abides by state practice act and guide for professional conduct
    • Establishes and maintains an effective working relationship with patients/families co workers other health care professionals and the community
    • Takes responsibility for own actions
    • Utilizes effective communication techniques
    • Applies effective problemsolving techniques
    • Assumes responsibility for professional growth and development
  • Clinical Application:
    • Upholds standards of care per policies/procedures
    • Meets requirements necessary to maintain licensure standards
    • Competent in standard areas of practice or has a plan for becoming competent
    • Abides by all regulatory standards
    • Utilizes outcome measures complies with established requirements and participates on PI committees
    • Participates in internal/external educational activities
    • Provides inservice on continuing education course attended to home facility
    • Assists with mentoring volunteers new hires observation students and student interns
    • Participates in journal club literature reviews case studies and/or product assessment
    • Participates in at least one community sponsored event.
  • Performs other duties as assigned.
  • Complies with all policies and standards.

Qualifications

  • Bachelors Degree in PT OT or ST as minimum requirement required
  • Masters Degree in PT OT or ST preferred
  • 24 years Experience working as a staff therapist required

Knowledge Skills and Abilities

  • Demonstrates thorough knowledge of human growth and development; anatomy physiology pharmacology and kinesiology; current methods techniques and purposes of physical therapy; the care and operation of physical therapy equipment
  • Requires strong analytical and organizational skills in order to review physicians orders assess functional status develop treatment plans evaluate progress and maintain related documentation
  • Demonstrates a high level of interpersonal and communication skills both verbal and written in order to establish and maintain positive and effective therapeutic/working relationships with patients families physicians and hospital staff
  • Demonstrates working knowledge of the English language verbal and written. Abides by the standards of practice as outlined by state and national associations.

Licenses and Certifications

  • PT Physical Therapist Current licensure in the state required
  • OT Occupational Therapist Registered Current licensure in the state required
  • ST Speech Therapist Current licensure in the state required
  • BCLS Basic Life Support required
