Job Description
We are seeking an experienced and professional Nurse Practitioner to join our team in Brooklyn NY. Responsibilities:
- Provide comprehensive healthcare services to patients in their homes.
- Conduct thorough assessments and develop appropriate treatment plans.
- Collaborate with other healthcare professionals to ensure continuity of care.
- Educate patients and their families on health management and disease prevention.
- Maintain accurate and uptodate patient records.
- Adhere to ethical and legal standards of nursing practice.
Qualifications:
- Current Nurse Practitioner license in New York.
- Minimum of 2 years of experience as a Nurse Practitioner.
- Strong clinical skills and knowledge of primary care practices.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate and patientcentered approach to care.
