Key Responsibilities New patients are screened and assessed through patient interview referral information and medical records and within established timeframe. Nutrition care plan is developed and implemented in collaboration with the healthcare team and in accordance with established standards. Documentation is up to date and complies with department standards. Diet orders are edited in accordance with the clinical privileges granted by the credentialing committee. Nutrition support plan is developed and implemented to progress patient toward established goals. Follow up care is provided per established standards of care. Patient and family counsel and education is provided to meet their level of understanding and prescribed diet regimen. Food/nutrition expertise is provided to interdisciplinary team to ensure patient and family needs are met. Discharge planning assistance is provided including referrals for post discharge nutritional care communication with outpatient dietician and/or home healthcare companies to ensure a smooth transition of care. Nutrition consultation for patients is completed within established timeframes. Patients compliance with diet plan is monitored. Liaison services are provided between food service and nutrition to ensure the meal service is efficient and in compliance with diet order. Clinical preceptor duties are carried out per established guidelines. Quality improvement efforts are participated in to meet department goals. Enteral formulary review assistance is provided to ensure that essential products and availability reflect consideration of current clinical practice and cost containment. Guidelines and therapeutic references relevant to nutrition care are updated to reflect industry best practices. Additional work activities are carried out to meet stated expectations.
