You will be updated with latest job alerts via emailA Nurse Practitioner (NP) on the Baker Service is a registered professional nurse with certification in an expanded role who provides direct care to individual and families in the inpatient surgical care settings. Under the guidance / supervision of the supervising surgeon and in collaboration with the resident house staff and attending surgeons the NP is responsible for the care of inpatients assigned to the Baker Surgical Service. The NP will work as an integral member of the patient care team to provide clinical services and communicate all of the relevant data with members of the team including obtaining patient histories and physical exams coordinating testing and consultations ordering and implementing treatment plans formulated on morning rounds communicating with attending surgeons other members of the care team RNs case managers and house staff referring physicians and patient families performing patient education facilitating necessary transfers dictating discharge summaries and preparing patients for discharge.This individual is responsible for coordinating care for multiple services both during the patients hospitalization and in the discharge setting. The NP plays a significant role in providing quality of care and continuity resulting in optimal and efficient healthcare delivery for patients and their families.
PRINCIPAL DUTIES AND RESPONSIBILITIES: Indicate key areas of responsibility major job duties special projects and key objectives for this position. These items should be evaluated throughout the year and included in the written annual evaluation.
1.0 Clinical Practice
1.1 Utilizes advanced nursing knowledge to identify prevent or solve complex and / or recurring patient care problems.
1.2 The nurse practitioner functions in direct collaboration with the physicians assigned to the team unit. Initiates facilitates and develops interdisciplinary collaboration.
1.3 Writes orders in accord with current clinical practice guidelines.
1.4 Obtains a detailed accurate medical history and performs a comprehensive physical examination on adult patients.
1.5 Reinforces information presented by the attending physician to the patient and / or family as necessary. Liasion to patient and family members; communicates updates and treatment plan.
1.6 Facilitates diagnostic tests consultations and procedures as indicated or requested by the attending surgeon. Arranges for prescription medications and educates patient and family as necessary.
1.7 Reviews laboratory and other diagnostic tests with appropriate action taken for abnormal results in collaboration with team/attending.
1.8 Rounds with team documents daily assessment and plan of care. Adjusts plan of care based on clinical changes and communicates updates with team. Writes notes on patients and discusses the care of each patient on the unit with the attending surgeon and other consult services involved in the care of the patient.
1.9 Prepares patients for discharge including patient and family discharge education planning continuing care and rehabilitation arrangements in collaboration with unit nursing staff arranging for discharge prescriptions and confirming primary care or referring physician follow up as appropriate.
1.10 Prescribes medications for patients in accordance with service prescribing policies and / or attending surgeon instructions.
1.11 Under the direct supervision of the attending surgeon executes a wide range of management plans for patients.
1.12 In collaboration with patients and families she/he designs a plan of care based on an assessment of physical emotional and social needs.
1.13 Streamlines care with multidisciplinary teams to ensure that team goals are being met.
1.14 Coordinates with appropriate consults facilitating optimal management of the patients plan
care.
1.15 Maintains patients right to privacy; assuring patient dignity in the provision of care.
1.16 Participates in educational meetings applicable to the service.
1.17 Identifies current trends in health care and their implication for nursing practice.

2.1 Serves as a resource to clinical staff in area(s) of expertise.
2.2 Participates in the development of formal and informal educational programs for unitbased
nursing staff.
2.3 Collaborates with unit staff clinical teachers clinical specialists and other healthcare
Professionals to meet staff and patient educational needs.
3.0 Outcomes
3.1 Collaborates with nurse managers clinical nurses case managers and medical staff to plan
implement and evaluate pertinent changes in practice and health care delivery.
3.2 Identifies systemwide factors contributing to undesirable outcomes such as complications
length of stay and overutilization of services.
3.3 Facilitates patient movement through the system and efficient use of resources though inter
departmental collaboration.
