One year nursing experience preferablyin a skilled nursing facility or community health setting.

The Licensed Practical Nurse(LPN) is a licensed nurse who participates in the plan of care to providecompetent compassionate and individualized nursing care to patients and theirfamilies as developed by a registered nurse or nurse practitioner. The LPN also participates in the medical planof care under the supervision of the physician or nurse practitioner and inactivities to facilitate safe and efficient practice operations.
Participates in the plan ofnursing care as developed by and under the supervision of a registered nurse ornurse practitioner
Communicates the plan of careand other pertinent information to/from other team members
Provides nursing careappropriate to age group of patients consistent with the practice scope ofservice
Observes reports anddocuments pertinent patient information in accordance with the standards ofdocumentation for ambulatory care nursing.
Performs nursing procedureswithin the scope of practice of an LPN and as outlined in the Nursing Proceduremanual.
Collects data to assist theregistered nurse or nurse practitioner to evaluate outcomes of nursing care forpatients and to modify the plan of care as appropriate.
Works with the registerednurse or nurse practitioner to define standards of excellence for patient care.
Participates in andcontributes to performance improvement activities.
Assures efficient patientflow through the office.
Assists with the patientintake process: assures patient recordcomplete prior to visit escorts patient to exam room and checksidentification obtains information about chief complaint reviews currentmedications and prescriptions needing refill obtains vital signsageappropriate measurements and other measurements as applicable reviewsrecord for immunization needs documents all of the above.
Instructs/assists patient toprepare for examination by provider including setting up special equipment asneeded.
Notifies provider whenpatient is ready and communicates pertinent information obtained during theintake process.
Assists patient throughcheckout process when exam is complete
Cleans and prepares room fornext patient
Responds to needs to expeditevisits and keep on schedule
Participates in the medicalplan of care under the direction of the physician or nurse practitioner
Assists provider with patientexamination procedures and treatments within the parameters defined by theDepartment of Nursing
Administers medicationswithin the parameters defined by the Department of Nursing after successfulorientation.
Collects & testsspecimens and instructs patient about specimen collection at home asapplicable.
Performs other testsprocedures and treatments within the LPN scope of practice.
Performs designated tasks totrack test results and communicate results to patients.
Receives patient messagesreviews with provider and takes action as directed.
Faxes mails or telephones inprescription renewals as directed by provider.
Assists provider in coordinatingcare with/making referrals to community agencies.
Participates in orientationof new permanent and temporary staff members.
Teaches and directs otherclinical support staff for whom she/he is responsible.
Appropriately utilizes MGHnursing resources.
Participates in dailyactivities conducive to the comfort and safety of patients their families andpractice staff.
Appropriately utilizesequipment and supplies.
Provides input to supervisorregarding practice needs.
Participates in practice andother committees for formulation of practice policies & procedures.
Professional development maintains and updates clinical knowledge and skills.
Collaborates with registerednurse or nurse practitioner to identify/meet own learning needs.
Participates in relevantcontinuing education programs.
Adheres to regulatoryrequirements for annual training.
