Qualifications:
1. Bachelors Degree in Nursing preferred
2. Graduate of NLN approved School of Nursing
3. Licensure as Registered Nurse in the Commonwealth of Massachusetts
4. 23 years experience in oncology nursing

Multidisciplinary Oncology Disease Center
Summary of Role and Responsibilities:
Establishes promotes and coordinates a collaborative and multidisciplinary
environment with physicians (medical oncologists surgeons radiation oncologists and hematology/oncology fellows) nurses support staff and other providers that is centered on the needs of the patients families and significant others. Acts as a liaison to provide and ensure continuity of care for the patient and family. Acts as clinical resource for financial navigator.
Program Responsibilities:
1. Participates in day to day operations and evaluation of the program.
2. Participates in program evaluation and development of strategic goals.
3. Attends team/program meetings.
4. Supports research activities in the program.
5. Serves as a program resource to staff and providers for program
information in response to outside requests.
6. Develops and participates in the QA/QI projects for the Oncology
Center.
Collaborates and coordinates:
1. Collaborates with and coordinates providers and support staff to facilitate appropriate decision making that will provide complete and comprehensive care to the patient and family.
2. Has the ability to triage and prioritize the needs of all patients in the disease center.
3. Must have effective communication skills in order to converse productively with patients families multidisciplinary team support staff and referring providers in the community.
4. Has the ability to designate and coordinate appropriate providers to the patient (i.e. fellows)
5. Collaborates on daily basis with financial navigator to provide clinical support and guidance for patients coming or evaluation and starting treatment.
6. Collaborated with new pt coordinator to ensure all records are obtained for clinician review.
Assessment:
1. Assessment begins with an initial phone call to the patient and family prior to their initial consultation. Obtains initial nursing assessment. This includes intake diagnosis history physical symptoms psychosocial assessment cultural assessment and educational needs.
2. Identifies potential treatment plan and critical events.
3. Determines which members of the multidisciplinary team will see patient on initial visit. Makes collaborative decision with physician leadership when needed.
4. Works with disease center staff to ensure that the patient is seen by the appropriate providers ie: radiation therapist surgeon medical oncologist.
5. Has the ability to determine if the patient may be a candidate for a clinical trial.
Intervention/Plan of Care:
1. Collaborates with physicians and providers to implement and manage the plan of care. May include scheduling testing treatment results reviewed and symptom management.
2. Considers patients total wellbeing and recommends appropriate referrals for the patient and family (i.e. social service nutrition case managementpalliative care)
3. Has the ability to triage urgent care and medical emergencies
Education:
1. Develops and implements teaching/information material for the Disease Center.
2. Educates patient and family regarding visit procedures disease treatments results and any other pertinent information.
3. Participates in Cancer Center wide education activities.
4. Develops and provides educational programs for nursing staff and support staff.
Documentation:
Utilizes established systems to document reason for visit testing ordered/completed (when appropriate) and referrals made and current medication list.
