Job Description

MA RNlicensure required. Certification required.

The Wound/Ostomy Nurse Clinician assumes roles of: clinician consultant change agent educator and participates in research activities. He/she provides expert clinical consultation and education to nurses family patient HCT regarding their responsibilities for coordinating all aspects of managing and delivering quality cost effective patient and family centered care specifically in the areas of wound ostomy and continence. The Wound/Ostomy Nurse Clinician collaborates with the nurse and physician to assure that all patients have a comprehensive plan of care that provides continuity across all settings. The Wound/Ostomy Nurse Clinician acts as a resource to the nurse in providing patient/family education. The Wound/Ostomy Nurse Clinician evaluates the success of care coordination and monitors the quality and continuity of care formally and informally through data analysis direct observation chart review one to one interaction and/or collaboration with the health care team Administrators Multidisciplinary Practice or Manager.Three to five years of relevant clinical experience is requiredBSN required and Completion of Debridement Workshoprequired.WCCrequired.MA RN licensure required.
