JOB DESCRIPTION
Our PostAcute team provides medical services for various SkilledNursing Rehabilitation Long Term Care and Assisted Living facilities.
The Attending Physician is responsible for managing health problems andcoordinating health care for patients in accordance with State and Federalrules and regulations and standards of care (assessment of health statusdiagnosis development of plan of care and treatment implementation of treatmentplan and evaluation of patient status).
Essential Job Functions:
- Competently perform exams and complete assessments andtreatment plan
- Perform History & Physical on all new admissions toskilled nursing facilities
- Adhere to Adfinitas policies for frequency of overseeingthe care of and reviewing the notes related to specific episodes of care forpatients of the skilled nursing facility.
- Facilitate the patients transition within and betweenhealth care settings e.g. admitting transferring and discharging patients.
- Collaborate with multidisciplinary team members.
- Supervise APPs as appropriate.
- Adhere to Adfinitas policies for frequency of patientvisits.
- Submit accurate and timely charges forall professional services performed
REQUIREMENTS
The ideal candidate will be geriatric fellowshiptrained or boarded in geriatrics. However we will consider any physician withprior geriatric experience or is open to working in the postacute setting.
- Board eligible/Board certified in Internal Medicine orFamily Medicine
- Highly effective verbal and written communication skillsincluding use of email are necessary in dealing with a variety of healthcareprofessionals Adfinitas colleagues family members and patients.
- Team oriented attitude toward patient care.
