Empath Home Health a member of Empath Health is seeking a Home Health Scheduler. The Home Health Scheduler is responsible for coordinating and maintaining clinical staff schedules using Empath scheduling software.
Location: Onsite in Sarasota FL
Schedule: MondayFriday 8:30am5pm
The Home Health Scheduler:
- Maintains clinical staff schedules and notifies staff if one is not present.
- Schedules client appointments/visits according to care plans/evaluations and staff availability.
- Enters scheduling data into Kinnser and creates schedules.
- Demonstrates professional conduct and ethics according to Empath Health policy and procedure.
- Maintains confidentiality of patient information and all Empath Health operations.
- Processes all incoming patient referrals from physicians and acute/nonacute facilities.
- Schedules clinical staff to clients determined by location and availability.
- Maintains clinical staff schedules and notifies staff if one is not present.
- Assists staff with coverage for PTO.
- Schedules client appointments/visits according to care plans/evaluations and staff availability.
- Enters scheduling data using Empath scheduling software to create schedules.
- Contacts care providers and clients regarding daytoday changes.
- Therapy visits are maintained with attention to 30day reevaluation and 13th/19th reevaluation to include multiple therapies.
- Performs general office duties including but not limited to word processing photocopies files shreds provides reception and telephone services.
- Verifies Medicare benefit eligibility through online DDE checks; and if necessary confirms beneficiarys status with other agencies by phone.
- Determines and communicates medical complexity/acuity of all referred patients and their orders to the Clinical Coordinator.
- Coordinates patient care with various members of the healthcare team including physicians physicians office staff hospital discharge planners/case managers and Territory Managers.
- Serves as a resource for physicians hospital personnel patients and family or caregivers in order to facilitate communication answer questions and resolve problems with assistance of Clinical Coordinator.
- Operates Kinnser DDE ECIN online to internet as needed Fax copy etc.
- Assists with scanning referrals and admission paperwork into the Kinnser system as needed.
- Performs other job duties and responsibilities as assigned by Empath leaders.
Requirements:
- At least one 1 year of experience in Home Health scheduling is preferred.
- High School Diploma or GED is required.
- Currently licensed as a Licensed Practical Nurse in Florida is preferred.
- Demonstrates strengths in collaboration and partnership to attain strategic goals business plan development and organization and followthrough.
- Possess organizational problemsolving time management and decisionmaking skills.
- Advanced interpersonal skills including verbal and written communication skills.
- Able to work autonomously with minimum supervision.
