CHS Behavioral HealthFellowship Overview: This is a twoyear fulltime fellowship with specialty emphases preparing fellows for independent practice in integrated primary care medical settings with diverse populations. Thetwice monthly didactics are designed to support fellows in their advancement of knowledge and skills in health psychology diversity professional development and career adjustment and leadership inhealthcare settings.Successful completion of this fellowship meets all requirements for licensure (LCSW or LPC) in thestate of Colorado.
Position Overview: Integrated Primary Care Family Medicine & Integrated Substance Treatment Services. The fellow will engage fully in clinic responsibilities providing consultation services for primary care providers crisis assessments diagnostic evaluations health promotion and behavior change interventions and brief timelimited psychotherapy. This position focuses on integrated primary care services throughout the lifespan and includes specialty training in substance treatment interventions in the context of primary care. The fellow will learn about Medication Assisted Treatments for addictions. The fellow will be placed in two Denver Health Primary Care clinics.

Job Summary
Clinician provides therapeutic services as a qualified licensure candidate under the direct supervision of a licensed behavioral health provider in the primary care medical setting. Clinical services include supervised diagnostic assessment and brief evidencebased interventions for behavioral health disorders cooccurring chronic medical conditions and substance use disorders. Clinician provides interventions focused on health behavior change with a multicultural lens and emphasis on the impact of social determinants of health on overall functioning. Clinician also provides case management and case coordination to facilitate referrals to specialty care as needed. Clinician functions as an integral member of the interdisciplinary team in the primary care medical setting as well as part of the larger organization. Clinician is actively involved in quality improvement data monitoring and in research projects. Clinician participates as a member of the Integrated Behavioral Health Division.
