Job Description
BAYADA Home Health Careis currently seeking an experiencedPhysical Therapist Assistant PTAfor a full time opportunity performingOutpatient home healthvisits for oursenior living office located in Moorestown NJ. This office services adult and geriatric clients on a per visit basis in territories throughout Burlington County and Camden County NJ.
As a home care Physical Therapist Assistant you will be an integral member of a multidisciplinary health care team that provides rehabilitative care and skilled nursing to clients affording them the opportunity to receive the medical care required to remain at home.

Responsibilities:
- Make outpatient visits to clients in designated geographic territories.
- Implement appropriate individualized care plans including manual therapeutic exercises gait training balance and other interventions.
- Participate with nursing in the multidisciplinary care plan as appropriate.
- Educate and instruct clients family members or other client representatives in rehabilitative care and activities necessary to promote the clients health safety and independent living.
- Accurately document observations interventions and evaluations pertaining to client care management and services provided utilizing a stateoftheart touch pad tablet.
Qualifications:
- Have current licensure or certification in the state ofNJ.
- Have graduated from a twoyear college level program approved by the American Physical Therapy Association as indicated by school transcript or diploma or
- Have two years of appropriate experience as an Physical Therapist Assistant (PTA) and a satisfactory grade on a proficiency exam conducted approved or sponsored by the U.S. Public Health Service when initial qualification occurred before December 31 1977
- Have at least one years work experience under the supervision of a qualified Physical Therapist.
- Other activities as requested.
