Job Description
We are currently seeking qualified candidates for a part time Social Worker position in the Las Vegasarea.
Job Description:
Providing social service activity as it relates to the patients and their families. Thisincludestreatment planning and consultation with other staff members. Experiencein assessing the patients and their familys emotional and social factors in orderto estimate their capacity and potential. Assisting in discharge planning from agency service when care is no longer appropriate by submitting a written summary of services provided. Thisincludesan assessment of the patients current statusto be retained in the patients chart. Participate in discharge planning and inservices programs related to the needs of the patient.
Qualifications:
- 1 yearsexperience Licensed Social Worker in the State of Nevada required
- Experience with Hospice or Home Health preferred
Minimum of two years experience in varied multidisciplinary settings preferred.
Must have current CPR or BCLS.
Must have current TB testing documentation and physical examination or within last 12 months.
Current State of Nevada drivers license and proof of auto insurance.
- Effective written and verbal communication
