We are seeking applicants for a Residential Medical Program Coordinator who interacts with participants staff caregivers physicians families behavior specialists case managers and other designated team members for a participant to ensure that the health/medical needs of the participants are met on a daily basis. The Residential Medical Program Coordinator provides an ongoing assessment of the participants of the Southwest Center (SWC) to ensure their health welfare and safety.

Schedule:
  • 8hour shifts
  • 11am to 7pm or 10am to 6:30pm
  • Shifts may include 1st 2nd 3rd weekends and holidays

Qualifications/Requirements:
  • Must have a high school diploma or its equivalent
  • Experience in working with children or adults with intellectual or developmental disabilities preferred.
  • Must be 18 years of age
  • Must pass a criminal background check
  • Must pass preemployment & random drug testing
  • Must have a valid drivers license and good driving and good driving record
  • Must have excellent communication and organization skills
  • Ability to maintain client confidentiality
  • Must be proficient in MS Office Suite for document creation editing and organization
  • Ability to multitask in a busy work environment.
  • Must have basic knowledge of typical office equipment & technology
  • A Certified Medical Assistant or Certified Nurses Assistant Certificate Preferred
Responsibilities:
  • Under the direction of the RN Residential Medical Program Manager will ensure and assist participants are taken to medical appointments as scheduled and all follow up instructions orders etc. are followed.
  • Will assist with training staff in Southwest Center medical policies.
  • Will assist with accompanying participants to the emergency room or other urgent care appointments as needed and as directed.
  • Interacts with participants staff caregivers physicians families behavior specialists case managers and other designated team members for a participant to ensure that the health/medical needs of the participants are met on a daily basis.
  • Will assist the RN Medical Program Manager and/or Residential Manager with checking medication administration records and ensuring medications are being administered correctly.
  • Communicates with physicians in securing signed orders as needed.
  • Evaluates all Medication Administration Records (MARs)weekly to ensure all information is current and no med errors have occurred.
  • Will assist RN Medical Program Manager with cycle fill each month as directed
  • Will update HRST of participants as directed
  • Assist with evaluating med errors when they occur developing plans to reduce them.
  • Will track supplies in residential homes an order needed supplies weekly.
  • Will be in on call rotation with the residential medical staff
  • Assist with completing medical house checks
  • Add medications to the MAR as needed by physician
  • Will keep medical binders in the residential homes updated regularly
  • Participates in the SWC Quality improvement Program.
  • Cooperates and works as a team member with coworkers and supervisors. Treats coworkers and supervisors with respect. Promotes teamwork and maintains a positive attitude.
  • Completes initial and annual training(CPR First Aid Tb skin assessment etc. as directed by Agency policy and regulatory standards. Attends monthly staff meetings and additional staff development trainings as scheduled.
  • Adheres to Agency policy and procedures. Maintains a consistent work attendance record. Limits use of calling off to a minimum and notifies direct supervisor in appropriate amount of time when unable to report to work.
  • Fosters collaborative working relationships among members of the leadership and management teams and programs.
