Job Description
It is essential for the successful candidate to have access to a car have a full drivers licence and be happy to transport customers in their car.
What is great about the role
You will play a key part in delivery of the preventative hubs in Llanelli Amman and Gwendreath these hubs provide preventative and solutionbased community support to support customers to improve and/ or maintain their health wellbeing and independence through social integration and strong community relationships.
As part of the lead team you will deliver the single point of contact from which customers can access information opportunities and support is visible accessible and available.
You will work closely with your team and partner organisations to deliver this service. You will receive referrals provide information have wellbeing conversations with customers deliver community engagement campaigns and develop community support networks.
What skills and experience do you need
We are looking for someone that is able to build great rapport and has experience of working with vulnerable people or in community development.
You will have the ability to plan deliver review and monitor a holistic package of support and have the ability to assess the needs of our customers and assess risk and propose risk management strategies.
For this role you will need to have a high standard of IT literacy and be keen to engage in further learning to continuously grow.

- The FTE salary for this position is 28041.00 FTE 22.5hrs per week 17051.96
- Work Pattern: 3 days per week but required to work a Wednesday to attend team meetings. Work pattern can be discussed during interview.
