The Personal Care Specialist supports the needs of patients by providing aide according to the patients plan of care while working under the direction of the assigned supervisor.
Essential Functions Statement(s)
- Provide care based on patients plan of care or service plan.
- Provide or assist with personal care including but not limited to bathing dressing oral hygiene hair care meal preparation feeding transfers and exercises based on the plan of care or service plan. Private duty includes pet care housekeeping errands shopping and other client assistance services.
- Maintain comfortable clean environment by adjusting lighting room temperature changing bed linens and light housekeeping as needed paying close attention to safety factors and accident prevention in the client home. (Client home refers to a residence within Independent Living (IL) building or a private home in the general community).
- Participate as a member of the interdisciplinary team and communicate changes in mental or physical status or other updates to appropriate individuals.
- Follow individualized plan of care documenting comprehensive observations and interventions in the EMR and communicate as necessary with the appropriate individual.
- Participate in continuous quality improvement activities.
- Precept and train new staff supporting professional relationships as required.
- Adhere to standard precautions and transmissionbased infection control procedures utilizing appropriate Personal Protective equipment.
- Follow all UCH policies and procedures as well as all applicable regulatory guidelines.
- Participate and comply with all deadlines for education requirements employee health screenings and attendance at meetings.
- Assist with scheduling private duty and nonskilled staff with clients in EMR
- Answer and make phone calls to clients related to personal care and nonskilled services
- Assist with office tasks such as documentation management service log reviews managing supplies etc.
- Assist with marketing private duty to prospective clients
- Complete private duty intakes with prospective clients
- Assist with covering calloffs when needed
- Perform all other duties as assigned.
Education / Certification / Licensure:
High School Graduate or General Education Development (GED): Preferred
State Tested Nursing Assistant with the State of Ohio or the
successful completion of the Certified Home Health Aid classes and certification exam: Preferred
CPR Certification: Preferred
Experience:
One 1 year previous experience preferred.
Computer Skills:
Must be able to proficiently use a computer the Internet and basic office equipment; Familiarity with Microsoft Office Software.
Other Requirements:
Must have a valid drivers license. Must be able to effectively perform essential duties while supporting UCHs mission vision and values in a professional and ethical manner. Must be able to adhere to all terms and conditions set forth in the United Church Homes Employee Handbook.
